Complaint Investigation Summary Report #: 5105 <br />COMPLAINT ID: COX2KGTDW Site Location: 14900 W HVVY 12, LODI CA, 95242 (DOCK ZUCCHINI #183) <br />Activity Summary <br />Recorded by <br />SUSZYCKI <br />SUSZYCKI <br />SUSZYCKI <br />SUSZYCKI <br />Filled out a prop. 65 form. <br />Abated the complaint as all violations have been corrected. Emailed the boat <br />owner and Jen Heckathorn the report and photo log. <br />Called Matt with Patriot Environmental and confirmed that clean-up and been <br />complete. Matt said they would wait for me to observe the location in person. <br />Visited the location to confirm the clean-up done by Patriot Environmental. Upon <br />arriving at the dock, I met with Matt Muslfo Patriot's project manager and two <br />representatives from the park. I observed the location around and near the boat to <br />be clean and free of visible contamination. No sheen or smell was noted, and fish <br />were seen swimming enthusiastically around the boat. <br />Mr. Muslfo provided a copy of the manifest, 021395466 JJK. Two 55-gallon drums <br />of liquid, non-RCRA hazardous waste (110 gallons water contaminated with <br />diesel) and seventeen 55-gallon drums (1,700 Lbs. of absorbent, booms, and <br />other solids contaminated by diesel) were listed on the manifest. Additionally, Mr. <br />Muslfo stated that the drums will be taken to a TSDF for proper storage. He also <br />estimates that about 32 gallons of diesel was released from the boat. Mr. Muslfo <br />mentioned that the spill occurred due to a damaged fuel line.
