DRAFT [TENTATIVE] WASTE DISCHARGE REQUIREMENTS ORDER R5-2023-OOXX 91 <br /> FORWARD, INC. <br /> FORWARD LANDFILL <br /> SAN JOAQUIN COUNTY <br /> STANDARD PROVISIONS & REPORTING REQUIREMENTS <br /> i. Results and Assessment for the Evaluation Monitoring <br /> Program. A report with the results and assessment based <br /> on the approved Evaluation Monitoring Program [Title 27, <br /> § 20425(b)]. <br /> ii. Updated Engineering Feasibility Study. An updated <br /> engineering feasibility study for corrective action based on <br /> the data collected to delineate the release and data from the <br /> ongoing monitoring program required under Title 27, <br /> section 20425(e) [Title 27, § 20425(c)]. <br /> iii. Amended ROWD for a Corrective Action Program. An <br /> amended report of waste discharge to establish a Corrective <br /> Action Program meeting the requirements of Title 27, section <br /> 20430 based on the data collected to delineate the release <br /> and based on the updated engineering feasibility study <br /> [Title 27, § 20425(d)]. <br /> g. The Discharger shall (for releases from MSW landfill units) discuss <br /> the results of the updated engineering feasibility study, prior to the <br /> final selection of a remedy, in a public meeting with interested and <br /> affected parties [40 C.F.R. § 258.56(d)]. <br /> K. General Provisions <br /> 1. In the event the Discharger does not comply or will be unable to comply <br /> with any prohibition or limitation of this Order for any reason, the <br /> Discharger shall notify the appropriate Central Valley Water Board office <br /> by telephone as soon as it or its agents have knowledge of such <br /> noncompliance or potential for noncompliance, and shall confirm this <br /> notification in writing within two weeks. The written notification shall state <br /> the nature, time, and cause of noncompliance, and shall describe the <br /> measures being taken to prevent recurrences and shall include a <br /> timetable for corrective actions. <br /> 2. All reports and transmittal letters shall be signed by persons identified <br /> below: <br /> a. For a corporation: by a principal executive officer of at least the <br /> level of senior vice-president. <br /> b. For a partnership or sole proprietorship: by a general partner or the <br /> proprietor. <br />
