Areas inspected: <br /> 2545 HAMMER LN.: Orthopedics (Exam Rooms. 20,17, 16,3 and CAST 6); Floor 2 Medical Specialties—Allergy (Exam <br /> Rooms 26, 27, 24, 22, 6 and Health Education); General Surgery—(Exam Rooms 14, 13, 15 & 11) Floor 3 Urology <br /> (Procedure Rooms 3 and 4, Exam Rooms 4, and 18); Cardiology(Exam Rooms 3 and 5) <br /> 2505 HAMMER LN.: Infusion (Exam Rooms 3, 5,1 and Storage); Oncology/Hematology and Dirty Utility, Lab (Draw <br /> Lab); Radiology (CTRoom, MRI Rooms are occupied), Janitor Closet, Pediatrics (Exam Rooms 4,5,and 6); Urgent Care <br /> (Exam Rooms 6,8, and 4); Janitor Closet at Urgent Care and Pediatrics. Family Mediicine (Exam Rooms 2,3, and 5). <br /> Internal Medicine ( Rm 2059, Exam Rooms 1,3,4,and 7); Women's Health OB/GYN (Exam Rooms 10 and 15, Room <br /> 2052, ultra sound 3, utility, and Quiet Zone)and 2nd Floor Storage Room. <br /> 7. Observed several short biohazard containers with hinged lids throughout the facility. Some containers lacked the <br /> biohazard labels on the backsides of the containers. In the future, provide labels on the backsides of the containers (as <br /> well as the sides and lids) as described in HSC 118280. <br /> 8. 1 observed ASTM D 1709 and ASTM D 1922 compliant bags. <br /> 9. 1 observed two Designated Storage areas, one for consolidated waste containers and one for biohazard containers. <br /> Corrections due by October 24, 2025 <br /> Reinspection on/about: A reinspection fee of$179 per hour may be charged.
