All violations must be corrected within specified timeframe. Violations that are classified as "MAJOR" pose an immediate threat to public health <br />and have the potential to cause foodborne illness. All major violations must be corrected immediately. Non-compliance may warrant immediate closure of <br />the food facility. <br /> #1 Demonstration of Knowledge <br />OBSERVATIONS: The 5 year Food Manager's certificate for this facility has expired. Renew the 5 year Food Safety <br />certificate within 60 days. Once renewed, provide a copy to Kadeanne Linhares by e-mail or text (klinhares@sjgov.org). <br />CALCODE DESCRIPTION: All food employees shall have adequate knowledge of and be trained in food safety as it relates to their <br />assigned duties. (113947) Food facilities that prepare, handle or serve non-prepackaged potentially hazardous food, shall have an <br />employee who has passed an approved food safety certification examination. (113947-113947.1) Any food handler hired after June 1, <br />2011 shall obtain a Food Handler Card within 30 days (113948). <br /> #29 Toxic Substances Properly Identified/Stored/Used <br />OBSERVATIONS: A can of Raid was observed on a shelf behind the arcade games. Discontinue the use of household <br />insecticides within the food facility. Discard or take home today. <br />CALCODE DESCRIPTION: All poisonous substances, detergents, bleaches, and cleaning compounds shall be stored separate from food, <br />utensils, packing material and food-contact surfaces. (114254, 114254.1, 114254.2) <br /> #33 Nonfood Contact Surfaces Clean <br />OBSERVATIONS: The back of the house shelving (between the pizza oven and the mop sink) needs to be cleaned. Clean <br />and sanitize today. <br />CALCODE DESCRIPTION: All nonfood contact surfaces of utensils and equipment shall be clean. (114115 (c)) <br /> #35 Equipment/Utensils Approved and in Good Repair <br />OBSERVATIONS: The top deck of the pizza oven has food accumulation. Clean under the top deck conveyor belt by 2 <br />weeks. <br />The dishmachine is not in use. Repair the dish machine to proper function or remove this piece of equipment from the <br />facility. <br />CALCODE DESCRIPTION: All utensils and equipment shall be fully operative and in good repair.
