DRAFT [TENTATIVE] WASTE DISCHARGE REQUIREMENTS ORDER R5-2023-00XX 98 <br />FORWARD, INC. <br />FORWARD LANDFILL <br />SAN JOAQUIN COUNTY <br />STANDARD PROVISIONS & REPORTING REQUIREMENTS <br />c.For a municipality, state, federal or other public agency: by either a <br />principal executive officer or ranking elected or appointed official. <br />d.A duly authorized representative of a person designated in a, b or c <br />above if: <br />i.The authorization is made in writing by a person described in <br />a, b, or c of this provision; <br />ii.The authorization specifies either an individual or a position <br />having responsibility for the overall operation of the <br />regulated facility or activity, such as the position of plant <br />manager, operator of a Unit, superintendent, or position of <br />equivalent responsibility (a duly authorized representative <br />may thus be either a named individual or any individual <br />occupying a named position); and <br />iii.The written authorization is submitted to the Central Valley <br />Water Board. <br />e.Any person signing a document under this Section shall make the <br />following certification: <br />I certify under penalty of law that I have personally <br />examined and am familiar with the information <br />submitted in this document and all attachments and <br />that, based on my inquiry of those individuals <br />immediately responsible for obtaining the information, <br />I believe that the information is true, accurate, and <br />complete. I am aware that there are significant <br />penalties for submitting false information, including the <br />possibility of fine and imprisonment. <br />3.The Discharger shall take all reasonable steps to minimize any adverse <br />impact to the waters of the State resulting from noncompliance with this <br />Order. Such steps shall include accelerated or additional monitoring as <br />necessary to determine the nature, extent, and impact of the <br />noncompliance. <br />4.The owner of the waste management facility shall have the continuing <br />responsibility to assure protection of waters of the state from discharged <br />wastes and from gases and leachate generated by discharged waste <br />during the active life, closure, and post-closure maintenance period of the
