A-1 <br /> ATTACHMENT A - INFORMATION SHEET <br /> GENERALIZED PERMIT APPLICATION PROCESS SUMMARY <br /> STATE WATER RESOURCES CONTROL BOARD <br /> ORDER WQ 2014-0153-DWQ <br /> GENERAL WASTE DISCHARGE REQUIREMENTS <br /> FOR SMALL DOMESTIC WASTEWATER TREATMENT SYSTEMS <br /> The Discharger shall perform the following: <br /> • Determine availability of regional collection system <br /> • Determine need to submit a Report of Waste Discharge (ROWD) <br /> • Review Nitrogen Effluent Limit Evaluation (Attachment 1) <br /> • Develop a Conceptual Wastewater Plan <br /> Contact Regional Water Board to discuss: <br /> • Wastewater characterization/treatment/disposal plan <br /> • Water balance precipitation value <br /> • Threat and Complexity/Application Fee <br /> • Storm water runoff/wastewater disinfection needed? <br /> • Review Nitrogen Effluent Limit Evaluation (Attachment 1) <br /> • Title 22 Engineering Report needed? <br /> • Groundwater monitoring needed? <br /> • Sanitary Sewer Overflow General Order Applicability <br /> • Operator Certification Required? <br /> • California Environmental Quality Act status <br /> • Electronic ROWD application/monitoring report procedures <br /> Submit Report of Waste Discharge <br /> (electronic submittal if requested by Regional Water Board) <br /> • Completed Form 200 <br /> • Application fee payment <br /> • Technical Report(prepared consistent with the guidance in Attachment B1, or as <br /> directed by Regional Water Board staff). <br /> jj <br /> Regional Water Board ROWD Review <br /> • If the ROWD is complete, a notice of applicability(NOA)will be prepared. <br /> • If incomplete, a Notice of Incomplete ROWD will be transmitted to the Discharger. <br /> NOA Issued <br /> • The NOA authorizes the wastewater discharge consistent with the ROWD, NOA, <br /> General Order, and title 22 Engineering Report(if applicable). <br /> • The NOA will specify compliance criteria, monitoring requirements, and reporting <br /> requirements.
