The secondary emergency <br />contact and the environmental contact are no longer at this facility. <br />REGULATION GUIDANCE: This information must be current and submitted as part of the business plan. <br />CORRECTIVE ACTION: Immediately log into the California Environmental Reporting System (CERS) at <br />http://cers.calepa.ca.gov/, enter the correct or updated information, and submit to the EHD for approval. <br />This is a minor violation. <br /> 5 HSC 25505(a)(1) Failed to electronically submit hazardous materials inventory for all reportable materials on site. <br />OBSERVATION 1: Two 275 gallon totes of Inspector's choice cleaner, two 500 gallon tanks of 3% sodium <br />hypochlorite, three 55 gallon drums of boiler MP 1812, three 55 gallon drums of paratherm Heat Transfer Fluid, a <br />275 gallon tote of sulfuric acid with water, and nalcolyte 8105 (coagulent) were found on site that have not been <br />reported to CERS. <br />OBSERVATION 2: The Hazardous Materials Inventory forms for Perasan MP-2 were not complete. Perasan A <br />replaces the chemical name of Perasan MP-2, CERS lists Perasan MP-2 as a solid, Perasan A is a liquid. <br />REGULATION GUIDANCE: Any material that meets or exceeds the reportable quantity shall be reported in the <br />facility's business plan. <br />CORRECTIVE ACTIONS: Immediately log into the California Environmental Reporting System (CERS) at <br />http://cers.calepa.ca.gov/, add the materials to the hazardous material inventory, and submit to the EHD for <br />approval. <br />This is a repeat violation, Class II. <br /> 6 HSC 25505(a)(2) Failed to electronically submit a site map with all required content. <br />OBSERVATION: The site map is incomplete. The site map was missing emergency shut-offs for the fuel and <br />propane tanks, the location of all the chemicals listed in the inventory, the exists and entrances to structures, and <br />emergency response equipment (fire extinguishers). <br />REGULATION GUIDANCE: A site map shall contain a north orientation, loading areas, internal roads, adjacent <br />streets, storm and sewer drains, access and exit points, emergency shutoffs, evacuation staging areas, hazardous <br />material handling and storage areas, and emergency response equipment. <br /> If a site map element is not applicable for your facility then list it on the map and label as “NA". <br />CORRECTIVE ACTION: Immediately log into the California Environmental Reporting System (CERS) at <br />http://cers.calepa.ca.gov/, upload the correct or updated information, and submit to the EHD for approval. <br />This is a repeat violation, Class II. <br />FA0009126 PR0519402 SC001 12/04/2023 <br />EHD 19-01 Rev. 12/06/2021 Hazardous Materials Program OIRPage 2 of 4 <br />1868 E.
