The secondary emergency <br />contact listed is no longer at the facility; not all the listed emergency equipment listed has a designated location (gas <br />cylinder leak repair kit, telephones, portable radios, automatic alarm chemical monitoring equipment. <br />REGULATION GUIDANCE: A business plan shall include the following emergency response procedures for a <br />release or threatened release of hazardous materials, including, but not limited to, the following: <br /> - immediate notification of local emergency personnel and the Environmental Health Department (EHD) <br /> - procedures for the mitigation of a release or threatened release to minimize any potential harm or damage to <br />persons, property, or the environment <br /> - evacuation plans and procedures, including immediate notice, for the site <br />CORRECTIVE ACTION: Immediately log into the California Environmental Reporting System (CERS) at <br />http://cers.calepa.ca.gov/, upload the correct or updated information, and submit to the EHD for approval. <br />This is a Class II violation. <br /> 9 HSC 25508.2 Failed to annually review and certify that the HMBP is complete and accurate by the due date. <br />OBSERVATION: The business plan information has not been reviewed and resubmitted in the California <br />Environmental Reporting System (CERS) annually by the due date, January 15. The HMBP was submitted <br />incomplete on 1/15/2018 for the year of 2018. The 2018 submittal was only submitted for the facility section. The <br />HMBP was submitted incomplete on 4/16/2019 for the year of 2019. The 2019 submittal was only submitted for the <br />facility and inventory section. The HMBP was submitted late on 3/6/2020 for the year of 2020. The HMBP was <br />submitted late on 3/1/2021 for the year of 2021. The HMBP was submitted incomplete on 2/17/2023 for the year of <br />2023. The 2023 submittal was only submitted for the facility section. A new submittal was made on 12/4/2023.
