See below: <br />1. On the business activities form, the facility answered "no" to having 1,320 gallons or more of aboveground <br />petroleum storage. During the inspection, the following aboveground containers were observed: 1,306 gallon diesel <br />generator, one 55 gallon drum of Frick 3 refrigeration oil, and one 55 gallon drum of waste refrigeration oil. <br />2. The owner/operator identification form lists Mason Miller as the secondary emergency contact. Per on-site <br />personnel, Mason's employment with the facility ended a few months ago. <br />REGULATION GUIDANCE: This information must be current and submitted as part of the business plan. <br />CORRECTIVE ACTION: Immediately log into the California Environmental Reporting System (CERS) at <br />http://cers.calepa.ca.gov/, enter the correct or updated information, and submit to the EHD for approval. <br />This is a minor violation. <br /> 6 HSC 25505(a)(2) Failed to electronically submit a site map with all required content. <br />OBSERVATION: The site map is incomplete. The following required items were missing: <br />-loading areas <br />-internal roads <br />-adjacent streets <br />-storm/sewer drains <br />-evacuation staging area <br />-locations of all emergency response equipment (eyewash/shower, fires extinguishers, spill kits) <br />-emergency shut-offs <br />-all access and exit points (for all structures, gates, etc) <br />REGULATION GUIDANCE: A site map shall contain a north orientation, loading areas, internal roads, adjacent <br />streets, storm and sewer drains, access and exit points, emergency shutoffs, evacuation staging areas, hazardous <br />material handling and storage areas, and emergency response equipment. <br /> If a site map element is not applicable for your facility then list it on the map and label as “NA". <br />CORRECTIVE ACTION: Immediately log into the California Environmental Reporting System (CERS) at <br />http://cers.calepa.ca.gov/, upload the correct or updated information, and submit to the EHD for approval. <br />This is a minor violation. <br />FA0010887 PR0520536 SC001 12/06/2023 <br />EHD 19-01 Rev. 12/06/2021 Hazardous Materials Program OIRPage 2 of 4 <br />1868 E.
