The monitoring <br /> plan lists the VR-409 as the annular sensor for the diesel tank, but it is utilizing the VR-343 annular sensor. <br /> REGULATION GUIDANCE:A copy of these monitoring plans shall be accessible on site at all times. <br /> CORRECTIVE ACTION: Immediately update the monitoring plan and retain a copy on site. <br /> This is a repeat violation, Class II. <br /> 208 CCR 2716(a-e) Failure to comply with the designated operator inspection requirements. <br /> OBSERVATION: The designated operator failed to check and note all required items on the visual inspection <br /> reports.The incorrect and missing information included the following from the inspection reports: <br /> -UDC 9/10 was not included in the visual inspection reports. <br /> -DO reports did not indicate that sumps with fuel alarms were visually inspected. <br /> -Secondary Containment Test Date is incorrect,the correct date is 01/15/2021. <br /> -DO indicated that having a qualified technician observe the sumps with fuel alarms was not applicable when there <br /> were fuel alarms. <br /> REGULATION GUIDANCE: During the visual inspection,the designated operator shall check that all required <br /> testing and maintenance for the UST system have been completed and shall check all required items on the report. <br /> CORRECTIVE ACTION: Ensure that designated operators performing visual inspections at this facility are including <br /> all of the required information on the reports. <br /> ------------------------------------------------------------------------ <br /> OBSERVATION: The designated operator failed to document all the alarms from the attached alarm history on the <br /> March, May and December designated operator inspection reports and failed to check that they were responded to <br /> appropriately. The missing alarms include: L1 87 STP sump setup/data warning on 02/27/2022,the L9 UDC <br /> setup/data warning on 04/17/2023, and the C1 diesel e85 fuel alarm on 04/19/2023. <br /> REGULATION GUIDANCE: During the inspection,the designated operator shall review the alarm history since the <br /> previous inspection, check that each alarm was documented and responded to appropriately, and attach a copy of <br /> the alarm history with documentation taken in response to any alarms to the inspection report. <br /> CORRECTIVE ACTION: Ensure that designated operators performing inspections at this facility are including all of <br /> the required information on the reports. <br /> This is a repeat violation, Class II. <br /> FA0003776 PR0231906 SCO01 01/11/2024 <br /> EHD 23-01 Rev.12/06/2021 Page 6 of 9 Underground Storage Tank Program OIR <br /> 1868 E.
