A NJ O A Q U l N Environmental Health Department <br /> COUNTY— <br /> Underground Storage Tank Program Inspection Report <br /> Facility Name: Facility Address: Date: <br /> KWIK SERV LODI BW 113* 420 W KETTLEMAN LN, LODI January 11, 2024 <br /> SUMMARY OF VIOLATIONS <br /> (CLASS I,CLASS II,or MINOR-Notice to Comply) <br /> Item# Remarks <br /> 214 CCR 2712 Failure to comply with UST operating permit conditions. <br /> OBSERVATION: The diesel 208 sensor was replaced on 12/11/2023 and a permit was not obtained from the EHD <br /> nor was the EHD notified. <br /> REGULATION GUIDANCE: Emergency repairs relating to UST system monitoring and release detection may be <br /> performed by a properly licensed,trained, and certified contractor pursuant to the EHD's policy and a permit <br /> obtained within one business day from the EHD. All other repairs must have a permit submitted to and approved by <br /> the EHD prior to beginning work. <br /> CORRECTIVE ACTION: Immediately obtain a permit from the EHD for the 208 sensor replacement. A"Failure to <br /> Obtain a Permit" penalty applies. <br /> This is a repeat violation, Class II. <br /> 303 CCR 2630(d), 2641(a) Failure to maintain piping to allow liquid to flow into sump(i.e.failure to remove test boots). <br /> OBSERVATION: The test boot in the E85 Turbine Sump was not pulled back and the Schrader valve was sealed. <br /> REGULATION GUIDANCE:Any potential release from the primary containment into the secondary containment in <br /> the E85 piping does not flow into a collection sump. All secondary containment systems shall be sloped so that all <br /> releases will flow to a collection sump located at the low point of the underground piping. <br /> CORRECTIVE ACTION: Immediately contact a properly licensed,trained, and certified contractor to ensure that any <br /> releases in the E85 piping flows into the E85 sump. <br /> This is a Class II violation. <br /> 306 HSC 25291(e)Failure to remove, analyze, and properly dispose of water in secondary containment(pre-Jul 2003). <br /> OBSERVATION: Liquid was observed in the Diesel STP sump, per Lakhvir Singh there is no means to remove from <br /> the sump and would require a contractor to remove water. <br /> REGULATION GUIDANCE: If water could enter into the secondary containment by precipitation or infiltration, it <br /> must be removed and disposed of properly. <br /> CORRECTIVE ACTION: Ensure that all sumps and annular spaces are maintained free of liquid. <br /> This is a repeat violation, Class II. <br /> FA0003776 PR0231906 SCO01 01/11/2024 <br /> EHD 23-01 Rev.12/06/2021 Page 7 of 9 Underground Storage Tank Program OIR <br /> 1868 E.
