A NJ O A Q U l N Environmental Health Department <br /> COUNTY— <br /> Underground Storage Tank Program Inspection Report <br /> Facility Name: Facility Address: Date: <br /> KWIK SERV LODI BW 113* 420 W KETTLEMAN LN, LODI January 11, 2024 <br /> SUMMARY OF VIOLATIONS <br /> (CLASS I,CLASS II,or MINOR-Notice to Comply) <br /> Item# Remarks <br /> 311 HSC 2636(a)(1), 25291(b) Monitoring system incapable of detecting leak into secondary containment(pre-Jul <br /> 2003). <br /> OBSERVATION: The E85/Diesel annular sensor failed to activate an audible and visual alarm when tested and, <br /> therefore,the tank is not monitored. <br /> REGULATION GUIDANCE: The UST system shall be designed and constructed with a monitoring system capable <br /> of detecting leaks from primary containment into secondary containment. <br /> CORRECTIVE ACTION: Immediately contact a properly licensed,trained, and certified contractor to replace the <br /> failed component with a functional component, and obtain a permit within one business day from the EHD, if <br /> needed. <br /> This is a Class II violation. <br /> 317 CCR 2636(f)(2) Failure of the line leak detector to detect a 3.0 gph leak at 10 psi and restrict or shut off flow. <br /> OBSERVATION: The Diesel line leak detector failed to detect a leak when tested. <br /> REGULATION GUIDANCE:All line leak detectors shall be capable of detecting a 3-gallon per hour leak at 10 psi. <br /> CORRECTIVE ACTION: Immediately have a properly licensed,trained, and certified contractor repair or replace the <br /> failed leak detector(LG 113-x listed, if applicable)and obtain a permit within one business day from the EHD, if <br /> needed. If the failed leak detector can't be replaced immediately,there is a possibility that the Diesel UST system <br /> may be red tagged to prevent fuel inputs. <br /> This is a repeat violation, Class II. <br /> 318 CCR 2635, 2637.1,2665, 2712 Failed to meet spill container requirements. <br /> OBSERVATION: The 91 direct-bury spill container failed when tested. <br /> REGULATION GUIDANCE:All spill containers shall have a minimum capacity of five gallons and be capable of <br /> containing a spill or overfill until it is detected or cleaned up. <br /> CORRECTIVE ACTION: Immediately discontinue deposition of petroleum into this tank until the component is <br /> repaired or replaced by a properly licensed, trained, and certified contractor under permit and inspection of the EHD. <br /> If the spill container can't be replaced immediately,there is a possibility that the 91 UST system may be red tagged <br /> to prevent fuel inputs. <br /> This is a Class II violation. <br /> Overall Inspection Comments: <br /> Complete and submit a copy of the Return to Compliance Certification form to the EHD with a statement <br /> documenting the corrective actions that have been or will be taken for each violation, and any supporting <br /> paperwork, by 02/11/2024 . <br /> To minimize person to person contact EHD is choosing to write the name of person receiving the report instead <br /> FA0003776 PR0231906 SCO01 01/11/2024 <br /> EHD 23-01 Rev.12/06/2021 Page 8 of 9 Underground Storage Tank Program OIR <br /> 1868 E.
