The most recent financial <br />responsibility documents in CERS are dated 2/6/2023. <br />- Financial Documents were not on site at time of inspection. <br />REGULATION GUIDANCE: Current financial responsibility documents are required to be submitted annually. <br />CORRECTIVE ACTION: Immediately log into the California Environmental Reporting System (CERS) at <br />http://cers.calepa.ca.gov/, and upload the required documents. <br />This is a repeat violation (Class 2) Violation. <br />56 <br />CA Code of Regulations 2716(a -e) 2030013 - DO inspections completed at least every 30 days and meet all <br />requirements (TCR 12). <br />OBSERVATION: The designated operator inspections for the following months were performed more than 30 days after <br />the previous inspection: <br />- Initial inspection conducted on 12/21/23, next inspection performed on 1/24/2024 (34 days). <br />REGULATION GUIDANCE: The designated operator visual inspections shall be conducted within 30 days or less of <br />the previous inspection. <br />CORRECTIVE ACTION: Ensure that all designated operator inspections are performed within the required time frames. <br />This is a repeat violation (Class 2) Violation. <br />58 <br />CA Code of Regulations 2715(c)(3), 2715(c)(4) 2010015 - Copies of Facility Employee Training Certificates maintained. <br />OBSERVATION: The Facility Employee Training Certificate was not available at time of inspection. <br />REGULATION GUIDANCE: All site employees must be trained prior to taking responsibility as a facility employee if <br />after October 1, 2018, or within 30 days of hire if prior to October 1, 2018. The Facility Employee Training Certificate <br />shall contain accurate information for all employees trained. <br />CORRECTIVE ACTION: Update the Facility Employee Training Certificate to contain the accurate information and <br />submit a copy of the updated form to the EHD. <br />This is a repeat violation (Class 2) Violation. <br />59 <br />CA Code of Regulations 2712(b)(1-2) 2030001 - Records of release detection monitoring and maintenance properly <br />maintained (TCR 9d). <br />OBSERVATION: Maintenance and monitoring records for the last three years were not found on site. <br />- At time of inspection, Monitoring System Certification for 2021 and 2023 were not on site. <br />REGULATION GUIDANCE: These records shall be maintained on site for at least three years.
