The following is an itemized list of underground storage tank violations that have not been addressed for MARCH LANE 76* as of February 13, 2024. Open violations from December 07, 2023 inspection Violation#208 -Failure to comply with the designated operator inspection requirements. 1. OBSERVATION: The designated operator failed to check and note the correct test dates for the Spill Containment Test on the designated operator inspection reports. The correct date for Spill Containment Test is 12/12/2022 but was listed as 12/22/2022 on multiple designated operator reports. REGULATION GUIDANCE: During the inspection, the designated operator shall check that all required testing and maintenance for the UST system have been completed and document the dates they were done. CORRECTIVE ACTION: Ensure that designated operators performing inspections at this facility are including all of the required information on the reports. 2. OBSERVATION The designated operator failed to check and note all required items on the visual inspection reports. The incorrect and missing information included the following from the inspection reports: - Multiple Designated Operator Reports did not have Section 5 completed for owner to respond to Designated Operator Compliance Issues listed in Section 3 -The 11/29/2023 Designated Operator Report did not have Section 7 and 8 completed and did not have Mechanical float mechanisms used in UDCs checked in Section 9 -The 11/02/2023 Designated Operator Report noted that there was liquid in UDC 9/10 in Section 3 but marked that UDC 9/10 was free of damage, water, debris and hazardous substances in Section 9 REGULATION GUIDANCE: During the visual inspection, the designated operator shall check that all required testing and maintenance for the UST system have been completed and shall check all required items on the report. CORRECTIVE ACTION: Ensure that designated operators performing visual inspections at this facility are including all of t e required information on the reports. OBSERVATION: The Facility Employee Training Certificate dated 11/2/2023, 6/14/2023, 4/18/2023, 3/21/2023 did not have accurate initial training dates or assuming responsibility ad—tes-- REGULATION GUIDANCE: All site employees must be trained prior to taking responsibility as a facility employee if after October 1, 2018, or within 30 days of hire if prior to October 1, 2018. The Facility Employee Training Certificate shall contain accurate information for all employees trained. CORRECTIVE ACTION: Update the Facility Employee Training Certificate to contain the accurate information and submit a copy of the updated form to the EHD. Page 3 of 4
