OBSERVATION: The facility failed to maintain records of inspections and tests in accordance with written procedures that the certifying engineer developed for the facility, including integrity tests, signed by the appropriate supervisor or inspector. 1-The annual and monthly inspections for the tanks were not completed as specified on table 4-2 of the facility SPCC Plan. The most recent annual inspection records for the following tanks: The 10,000 gallons diesel tank was done on 2/04/2021 The 2,000 gallons diesel tank was done on 11/20/2020 The day-tank#1 stationary standby generator was done on 1/20/2023 The day-tank#2 stationary standby generator was done on 10/20/2020 The day-tank#3 stationary standby generator was done on 10/20/2020 The day-tank#4 for stationary standby generator was done on 11/20/2020 2-There were no records for the annual inspections for the 5,000 gallons gasoline tank 3-There were no records for the monthly and annual inspections for the following tanks: The 660 gallons diesel tank for stationary standby generator The 454 gallons diesel tank non-op stationary standby generator The 200 gallons diesel tank for stationary standby generator The 150 gallons used oil tank for vehicle maintenance The 120 gallons of motor oil for vehicle maintenance REGULATION GUIDANCE: 112.7(e)Conduct inspections and tests required by this part in accordance with written procedures that you or the certifying engineer develop for the facility.You must keep these written procedures and a record of the inspections and tests, signed by the appropriate supervisor or inspector,with the SPCC Plan for a period of three years. Records of inspections and tests kept under usual and customary business practices will suffice for purposes of this paragraph. 112.8(c)(6)Test or inspect each aboveground container for integrity on a regular schedule and whenever you make material repairs.You must determine, in accordance with industry standards,the appropriate qualifications for personnel performing tests and inspections,the frequency and type of testing and inspections,which take into account container size, configuration, and design (such as containers that are: shop-built,field-erected, skid-mounted, elevated, equipped with a liner, double-walled, or partially buried). Examples of these integrity tests include, but are not limited to: visual inspection, hydrostatic testing, radiographic testing, ultrasonic testing, acoustic emissions testing, or other systems of non-destructive testing.You must keep comparison records, and you must also inspect the container's supports and foundations. In addition, you must frequently inspect the outside of the container for signs of deterioration, discharges, or accumulation of oil inside diked areas. Records of inspections and tests kept under usual and customary business practices satisfy the recordkeeping requirements of this paragraph. CORRECTIVE ACTION: Immediately begin conducting all inspections and tests in accordance with the procedures outlined in the SPCC Plan and begin using forms provided by the SPCC plan. Keep written inspection procedures and records of inspections and testing signed by the appropriate supervisor/inspector for at least three years. Submit proof of correction to the EHD.
