The facility could <br />not demonstrate that any training had been given to employees. In addition, used oil was observed spilled on the <br />ground with a leaking container. <br />REGULATION GUIDANCE: A Hazardous Materials Business Plan (HMBP) shall be established and implemented <br />for emergency response to a release or threatened release of a hazardous material when storing hazardous <br />materials at or above the thresholds quantities of 55 gallons for materials that are liquids, 500 pounds for solids, or <br />200 cubic feet for compressed gas, at standard temperature and pressure. <br />CORRECTIVE ACTION: Immediately log into the California Environmental Reporting System (CERS) at <br />http://cers.calepa.ca.gov/, enter the required information, and submit for review by the EHD. <br />This is a repeat violation, Class II. <br /> 4 HSC 25508(a)(1) Failed to electronically submit the Business Activities or Owner/Operator Identification page. <br />OBSERVATION: The Business Owner/Operator Identification form was not complete. <br />The following issues were observed: <br />- The facility marked "No" to hazardous waste generation. Hazardous waste was observed on site. <br />- The facility marked "Yes" to "does your facility need to report the closure/removal of a tank that was classified as <br />hazardous waste and cleaned on-site?" During the inspection, nothing was observed regarding the closure of a <br />tank that was classified as hazardous waste and cleaned on-site. <br />- The facility marked "Yes" to having more than 1320 gallons of total petroleum product for aboveground petroleum <br />storage.
