HSC 117975 - LQG shall maintain treatment and tracking documents for 2 years. <br /> OBSERVATIONS: <br /> 1. During inspection, review of the tracking and treatment documents was requested. No tracking and treatment <br /> documents were able to be provided during the inspection. <br /> CORRECTIVE ACTIONS: <br /> 1. Please provide tracking and treatment documents for medical waste for the past 2 years for LEA review. <br /> 24. HSC 118275(a)(3) and (4)—(3) Sharps waste, including sharps and pharmaceutical waste containerized pursuant to <br /> paragraph (7)shall be contained in a USFDA approved sharps container that meets USDFA labeling requirements and is <br /> handles pursuant to section 118285. (4)Trace chemotherapy waste, as defined in paragraph (5)of subdivision (b) of Section <br /> 117690, shall be segregated for storage, and, when placed in a secondary container, that container shall be labeled with the <br /> words "Chemotherapy Waste," "CHEMO," or other label approved by the department on the lid and sides, so as to be visible <br /> from any lateral direction, to ensure treatment of the biohazardous waste pursuant to Section 118222. <br /> OBSERVATIONS: <br /> 1. Mixed waste was observed within sharp containers. <br /> 2. Chemotherapy waste container in designated accumulation area had labels that were peeling and illegible. <br /> CORRECTIVE ACTIONS: <br /> 1. Provide training to all necessary employees to assure the proper segregation of medical waste from all other waste. <br /> Provide proof of training to EHD. <br /> 2. Provide adequate labels to chemotherapy waste container in designated accumulation area. <br /> 25. HSC 118280(c)— Biohazard waste shall be bagged in accordance with subdivision (b) of section 118275. Containers shall <br /> be leak resistant, have tight-fitting covers, and be kept clean and in good repair. Containers may be of any color and shall be <br /> labeled with the word "BIOHAZARD"or"Biohazardous Waste" on the lid and sides to be visible from any lateral direction. <br /> Containers shall comply with United States Department of Transportation requirements when prepared for transport offsite from <br /> the facility. <br /> OBSERVATIONS: <br /> 1. At the designated accumulation area, the red biohazard container did not have any biohazard labels. <br /> 2. Medium sized containers with spin top lids were open. Per operator, lid has a seal that will permanently close the <br /> container if lid is closed all the way. <br /> CORRECTIVE ACTIONS: <br /> 1. Please provide the proper biohazard labels to the lid and all sides of the container to comply with section HSC 118280. <br /> Provide proof of correction to EHD. <br /> 2. Please provide containers that have tight fitting covers to mitigate safety risk. Provide proof of correction to EHD. <br /> 30. HSC 118310—A designated accumulation area used for the storage of medical waste containers prior to transportation or <br /> treatment shall be secured so as to deny access to unauthorized persons and shall be marked with warning signs on or adjacent <br /> to, the exterior of entry doors, gates, or receptacle lids. The wording of warning signs shall be in English, "CAUTION— <br /> BIOHAZARDOUS WASTE STORAGE AREA—UNAUTHORIZED PERSONS KEEP OUT," and in Spanish, "CUIDADO—ZONA <br /> Reinspection on/about: A reinspection fee of$179 per hour may be charged.
