Designated accumulation area did not have a lock or proper signage to prevent unauthorized access. <br /> CORRECTIVE ACTIONS: <br /> 1. Provide proper signage and security to the door of the designated accumulation area to prevent access from any <br /> unauthorized persons. <br /> 31. HSC 117970(d) — Generators shall update their medical waste management plant, established pursuant to section 117960, <br /> when any of the information in the plan changes and shall have the plan on file for review during inspection or upon request. The <br /> updated plan shall be submitted within 30 days of the change. <br /> OBSERVATIONS: <br /> 1. Facility did not provide the Medical Waste Management Plan for review during inspection. <br /> CORRECTIVE ACTIONS: <br /> 1. Provide a copy of the Medical Waste Management Plan to keep onsite during inspection. Please provide proof of <br /> correction to EHD. <br /> 32. HSC 117630(c) — Biohazard bag color shall be red, except when yellow bags are used to further segregate trace <br /> chemotherapy waste. <br /> OBSERVATIONS: <br /> 1. Chemotherapy container in bioaccumulation container was lined with a red bag and not a yellow bag. <br /> CORRECTIVE ACTIONS: <br /> 1. Provide proper colored bags for the trace chemotherapy bins within the facility. Provide proof of correction to EHD. <br /> Corrections due to EHD by 8/26/25 <br /> Notes: <br /> 1. Inspection report emailed to Sheila Sexton at ssexton(a)..shomg.net <br /> 2. No tracking and treatment documents reviewed. Facility personnel did not have ability to provide documentation. <br /> 3. No MWMP plan onsite. <br /> 4. Sheila Sexton was not available for inspection. Inspection conducted with Dominique Serros (Pharmacy Technician). <br /> 5. Barnett Medical, according to pharmacy tech, continues to be the medical waste hauler for this site. <br /> Reinspection on/about: A reinspection fee of$179 per hour may be charged.
