HSC 117975-LQG shall maintain treatment and tracking documents for 2 years. A large <br /> quantity medical waste generator required to register pursuant to this chapter shall maintain <br /> or a minimum of two years individual treatment records and the tracking document for all <br /> untreated medical waste shipped offsite for treatment. <br /> OBSERVATION: The tracking and treatment record from 3-7-22 was not available for review <br /> since the hauler Stericycle does not have a record for that date. <br /> CORRECTIVE ACTION: Provide the record or explain how the site will be sure to store <br /> biohazardous waste for no more than seven days per HSC 118280(e)(1)(A). <br /> RESPONSE: <br /> A shipment record from March 7, 2022,was not available from Stericycle, our regulated medical waste <br /> hauler,because a pick-up did not occur on that date. As a result of a previous audit performed at the Red <br /> Cross fixed site on March Lane, Stockton, CA on December 18, 2022, corrective actions were put into <br /> place to address missing shipment records. The corrective actions were added to attachment IV of the <br /> California Regulated Medical Waste Program (v2.0) and implemented on January 18, 2023. As <br /> indicated in attachment IV, if the vendor misses a waste pickup, Red Cross staff are required to contact <br /> the vendor and determine if a new pick-up will be scheduled and the reason for the missed pick-up. This <br /> discussion is then documented in an email to the vendor and a copy of this email is to be maintained with <br /> all the shipping records for the location. Because this issue occurred prior to the corrective actions, <br /> documentation of contact with Stericycle is not available. <br /> 25. HSC 118280-Biohazard bags shall be containerized and stored pursuant to this section. (A)If the <br /> person generates 20 or more pounds of biohazardous waste per month, the person shall not contain or <br /> store that waste above 0° <br /> Centigrade(30'Fahrenheit) at an onsite location for more than seven days without obtaining prior <br /> written approval of the enforcement agency <br /> OBSERVATION:Tracking and treatment records were observed for 12-6-2021 and then the <br /> next pick up was 12-23-2021. CORRECTIVE A CTION: Provide tracking and treatment <br /> documentation of pick up on 12-13-2021. <br /> RESPONSE: <br /> See the response to Observation 13. Corrective actions have been added to the California Regulated <br /> Medical Waste Program to address similar issues identified after January 18, 2023. <br /> Page 2 of 3 <br />
