Please be <br /> aware that all EHD staff time associated with a reinspection will be billed at our current hourly rate ($156). <br /> This is a repeat violation (Class 2) Violation. <br /> 53 CA Health and Safety Code 25292.2; CCR 2711(a)(11)2010007-Submitted/maintained current Cert of Financial <br /> Responsibility/mech of financial assurance(TCR 11). <br /> OBSERVATION: Financial responsibility documents have not been submitted to the EHD.The most recent financial <br /> responsibility documents in CERS are dated 11/8/2021. <br /> REGULATION GUIDANCE: Current financial responsibility documents are required to be submitted annually. <br /> CORRECTIVE ACTION: Immediately log into the California Environmental Reporting System (CERS)at <br /> http://cers.calepa.ca.gov/, and upload the required documents. <br /> This is a repeat violation (Class 2) Violation. <br /> 56 CA Code of Regulations 2716(a-e)2030013-DO inspections completed at least every 30 days and meet all <br /> requirements(TCR 12). <br /> 1)OBSERVATION:The designated operator failed to check and note the correct test dates for the Secondary <br /> Containment Test on the 8/25/2023 designated operator inspection report. A date was not listed on this report. The <br /> correct date for Secondary Containment Test is 2/15/2023 <br /> REGULATION GUIDANCE: During the inspection, the designated operator shall check that all required testing and <br /> maintenance for the UST system have been completed and document the dates they were done. <br /> CORRECTIVE ACTION: Ensure that designated operators performing inspections at this facility are including all of the <br /> required information on the reports. <br /> ------------------------------------------------------------------------- <br /> 2)OBSERVATION: Designated operator reports were not signed within the required time frame. <br /> -The 9/22/2023 designated operator report was signed by the owner/operator on 9/29/2023 <br /> -The 5/31/2024 designated operator report was not signed or dated by the owner/operator <br /> REGULATORY GUIDANCE:Within 48 hours of the completion of the designated UST operator visual inspection,the <br /> designated operator shall sign and provide the owner or operator with a copy of the inspection report.
