• 0 <br />PURSUANT TO PRC SECTIONS 45000 and 45005. ZADOR. ERVIN & N ARE <br />2 ORDERED TO TAKE THE FOLLOWING CORRECTIVE ACTION: <br />3 <br />4 1. Contact the California Waste Management Board (CIWMB) at 1-866-896-0600 to obtair. <br />5 a Tire Program Identification Number (TPID #). <br />6 2. Remove accumulated solid waste and waste tires from site. Contact a registered waste tir <br />7 hauler to remove waste tires. Material suitable for recycling shall be recycled at an <br />8 appropriate recycle facility. COPIES OF ALL DISPOSAL RECEIPTS AND WASTE <br />9 TIRE MANIFESTS OR COMPREHENSIVE TRIP LOGS SHALL BE PROVIDED <br />10 TO THIS DEPARTMENT. <br />11 <br />12 3. An application to determine if tires on site used for Firing Range backdrop will be <br />13 considered a beneficial reuse must be made in writing to the Local Enforcement Agency <br />14 (LEA) and the California Integrated Waste Management Board (CIWMB). <br />15 <br />16 4. Submit to this office, for review and approval, a corrective action plan to re -grade the site <br />17 and to provide adequate cover to correct areas of exposed solid waste. Contact Regional <br />18 Water Quality Control Board (Region 5) to determine monitoring requirements for thi,, <br />19 site. <br />20 <br />21 5. The contents of the corrective action plan shall include, but not be limited to: <br />22 a site health and safety plan, a scope of work, work -plan, the identification of the <br />23 contractor performing the remediation, the schedule for corrective action measures, <br />24 a plan for handling and disposal of any household hazardous wastes and/ or hazardous <br />25 wastes should they be discovered, and a final site drainage and erosion control plan. <br />26 material suitable for recycling shall be recycled at an appropriate recycle facility. <br />27 COPIES OF ALL DISPOSAL RECEIPTS SHALL BE PROVIDED TO THIS <br />-5- <br />
