Provide an invoice and sterilization certificate for the needles and instruments used by Casey Price. A photograph of the lot# of the instrument should also be submitted. I have a folder with all of the supplies that the artists use in the building. the following link is where all of the sterilization certificates can be seen. Post Bloodborne Pathogen Training Certificate at the procedure areas. Provide a photograph showing the BBP certificates are posted at a conspicuous place. All BBP certificates are posted in the procedure area. Pictures of such are attached. 3. The consent form and medical questionnaire should be updated to include the information from Form B. Submit an updated copy of the consent form and medical questionnaire to the EHD. I am unsure as to what details that you want included in the consent form as far as the expectations post procedure. Would you be able to steer me in the right direction? 4. The signs and symptoms of infection should be included on the piercing aftercare instructions. Update the aftercare instructions and provide a copy to the EHD. - this is something that we have to have printed and delivered to us premade. They have been submitted and the new pamphlets will have the above information. As soon as I take delivery on those I will submit a picture. In the meantime, we are handing out a printed piece of paper with such instructions. Picture is attached. 5. Repair the hole and peeled paint in the piercing room. Also, repaint the 3rd procedure area. Submit photographic evidence to the EHD. Picture attached showing the repairs of the hole. 6. Provide a copy of an updated Infection Prevention Control Plan to the EHD. Ensure all practitioners are trained on the IPCP, provide EHD with evidence of training (such as a sign in sheet). Attached is a picture of the sign in sheet of training that was conducted. i
