The consent form and medical questionnaire should be updated to include the information from Form B. <br /> Submit an updated copy of the consent form and medical questionnaire to the EHD. <br /> 4. The signs and symptoms of infection should be included on the piercing aftercare instructions. Update the <br /> aftercare instructions and provide a copy to the EHD. <br /> 5. Repair the hole and peeled paint in the piercing room. Also, repaint the 3`d procedure area. Submit <br /> photographic evidence to the EHD. <br /> 6. Provide a copy of an updated Infection Prevention Control Plan to the EHD.
