0 FACILITIES USING REUSABLE EQUIPMENT AND D&AMINA'nON AND STERILIZATION PROCEDURES <br /> Describe the location of gloves available within your facility: Each procedure station and <br /> sterilization room will be supplied with nitrile gloves and stored in a clean holder within <br /> reach of the operator. <br /> Describe the use of barrier film, dental wraps, absorbent pads,paper towels, aprons, bibs, and <br /> any film used in your facility prior to the performance of body art and describe what equipment is <br /> covered and with what type of barrier is used in each instance: <br /> Barrier film is used to protect surfaces of pwer supplies and adjustable lighting. Dental <br /> bibs are used to cover procedural surfaces and to absorb any excess ink or water that <br /> may occur during procedure. Bibs are also used to protect piercing area. Plastic wrap <br /> disposable sheets will be used to cover all procedure tables or chairs that come in <br /> conlM with cu=mer during procedure. <br /> How will skin be prepared prior to the procedure? If skin at the procedure site is to be shaved, <br /> describe the solution used to prepare the skin, type of razor, and the method of razor disposal: <br /> Skin is prepped with isopropyl alcohol prior to shaving or marking for piercing. A <br /> disposable razor is used to shave the area and d' of in the sharps container after <br /> use. <br /> What solution or transfer agent is used to apply stencils or mark work sites? <br /> Tattoo/Permanent Make Up: Green soap or Stencil Stuff to transfer <br /> Piercing or Branding: Gentian Violet or single use skin scribes. <br /> When covering a procedure site, a sterile dressing must be used. <br /> What type of sterile dressing (s) are used?
