Environmental Health Department <br />CP22 - HMBP Inspection Report <br />Date: <br />September 18, 2024 <br /> Facility Address: <br /> 1801 E DR MARTIN LUTHER KING JR BLVD, <br /> Facility Name: <br /> Gillson Trucking Inc . <br />SUMMARY OF VIOLATIONS <br />(CLASS I, CLASS II, or MINOR - Notice to Comply) <br />RemarksItem # <br />REGULATION GUIDANCE: Within 30 days of one the following events, business shall electronically update the <br />information submitted to the California Environmental Reporting System (CERS): a) A 100 percent or more <br />increase in the quantity of a previously disclosed material. b) Any handling of a previously undisclosed hazardous <br />materials c) A change of business or facility address d ) Change business ownership e) Change of <br />business name f)(1) A substantial change in the handler 's operations occurs that requires modification to any <br />portion of the business plan. (2) For the purpose of this subdivision, “substantial change” means any change in <br />a facility that would inhibit immediate response during an emergency by either site personnel or emergency response <br />personnel, or that could inhibit the handler's ability to comply with Section 25507 , change the operational <br />knowledge of the facility, or impede implementation of the business plan. <br />CORRECTIVE ACTION: Immediately log into the California Environmental Reporting System (CERS) at <br />http://cers.calepa.ca.gov/, enter the correct or updated information, and submit to the EHD for approval. <br />This is (Class 2) Violation. <br />CA Health and Safety Code 25508(a)(1) 1010003 - Business Owner/Operator ID and Business Activities pages <br />electronically submitted and are accurate. <br />OBSERVATION: The Business Owner/Operator Identification form was not complete. The inaccurate information is: <br />The facility marked yes to CalARP. The facility has no record of CalARP materials on site . <br />REGULATION GUIDANCE: This information must be current and submitted as part of the business plan. <br />CORRECTIVE ACTION: Immediately log into the California Environmental Reporting System (CERS) at <br />http://cers.calepa.ca.gov/, enter the correct or updated information, and submit to the EHD for approval. <br />This is (Minor) Violation. <br /> 9 <br />CA Health and Safety Code 25505(a)(1) 1010004 - Complete and accurate Hazardous Materials Inventory information <br />electronically submitted. <br />OBSERVATION: One 55 gallons of gear oil, 55 gallons transmission oil, 55 gallons of acetic acid 80%, and 55 gallons <br />of electrolyte for used car batteries, four 55 gallons cleaner degreaser, four 55 gallons aluminum brighter were found <br />on site that have not been reported in CERS. <br />REGULATION GUIDANCE: Any material that meets or exceeds the reportable quantity shall be reported in the <br />facility's business plan. <br />CORRECTIVE ACTIONS: Immediately log into the California Environmental Reporting System (CERS) at <br />http://cers.calepa.ca.gov/, add the materials to the hazardous material inventory, and submit to the EHD for approval. <br />This is a repeat violation (Class 2) Violation. <br /> 10 <br />CA Health and Safety Code 25505(a)(2) 1010005 - Site Map with all required content electronically submitted. <br />OBSERVATION: The site map is incomplete.
