Environmental Health Department <br />CP22 - HMBP Inspection Report <br />Date: <br />September 18, 2024 <br /> Facility Address: <br /> 1801 E DR MARTIN LUTHER KING JR BLVD, <br /> Facility Name: <br /> Gillson Trucking Inc . <br />SUMMARY OF VIOLATIONS <br />(CLASS I, CLASS II, or MINOR - Notice to Comply) <br />RemarksItem # <br />present, loading areas, internal roads, storm and sewer drains, and emergency shutoffs , as well as additional map <br />requirements the governing body of the unified program agency finds necessary. <br />CORRECTIVE ACTION: Immediately log into the California Environmental Reporting System (CERS) at <br />http://cers.calepa.ca.gov/, upload the correct or updated information, and submit to the EHD for approval. <br />This is a repeat violation (Class 2) Violation. <br />CA Health and Safety Code 25505(a)(4) 1020002 - Provided initial and annual training and maintained training records <br />for a minimum of three years. <br />OBSERVATION: Employee training records were not available for review at time of inspection. <br />REGULATION GUIDANCE: The business plan shall include provisions for ensuring that appropriate personnel receive <br />initial and annual refresher training. All employees shall be trained in safety procedures in the event of a release or <br />threatened release of a hazardous material, including, but not limited to, familiarity with the following plans and <br />procedures: (A) Immediate notification contacts to the appropriate local emergency response personnel and to the <br />unified program agency. (B) Procedures for the mitigation of a release or threatened release to minimize any potential <br />harm or damage to persons, property, or the environment. (C) Evacuation plans and procedures, including immediate <br />notice, for the business site. This training shall be documented electronically or by hard copy and shall be made <br />available for a minimum of three years. <br />CORRECTIVE ACTION: Immediately provide employee training for appropriate personnel and submit a copy of the <br />training records to the EHD. <br />This is a repeat violation (Class 2) Violation. <br /> 14 <br />CA Health and Safety Code 25404.1.2 1010 - Administration/Documentation - General. <br />OBSERVATION: This facility was inspected on 11/09/2023 and violations were written: The return to compliance was <br />expected on 12/09/2023. The inspection report was delivered to this facility on 11/09/2023. A complete Return to <br />Compliance Certification form was not received by the San Joaquin County Environmental Health Department (EHD) <br />within 30 days of inspection report delivery. <br />REGULATION GUIDANCE: (c) (1) A person who receives a notice to comply detailing a minor violation shall have not <br />more than 30 days from the date of the notice to comply in which to correct any violation cited in the notice to comply. <br />Within five working days of correcting the violation, the person cited or an authorized representative shall sign the <br />notice to comply, certifying that any violation has been corrected, and return the notice to the San Joaquin County <br />Environmental Health Department (EHD). (e) If a person who receives a notice to comply pursuant to subdivision (a) <br />disagrees with one or more of the alleged violations listed on the notice to comply, the person shall provide the EHD a <br />written notice of disagreement along with the returned signed notice to comply. If the person disagrees with all of the <br />alleged violations, the written notice of disagreement shall be returned in lieu of the signed certification of correction <br />within 30 days of the date of issuance of the notice to comply. <br />CORRECTIVE ACTION: Submit a corrective action statement to the EHD within 30 days. <br />This is a repeat violation (Class 2) Violation. <br /> 19 <br />Overall Inspection Comments: <br />FA0013638 PR0521042 SC001 09/18/2024 <br />EHD Rev. 12/06/2021 CP22 - HMBP OIRPage 4 of 5 <br />1868 E.
