Site Health and Safety Plan Page 38 <br />10.6 Evacuation Routes and Procedures <br />Site work that shall always be aware of evacuation routes and procedures. Appropriate <br />primary and alternate evacuation routes and assembly area shall be marked on the site <br />map and updated as needed. The roots and assembly areas will be determined by conditions <br />at the site at the time of evacuation based on wind direction, the location of the hazard <br />source, and other factors as determined by rehearsals and input from site personnel. <br />If an evacuation notice is given, site workers shall immediately leave the work site with their <br />respective parties, if possible, by way of the nearest exit. Emergency decontamination <br />procedures detailed later in this section shall be followed, to the extent practical, without <br />compromising the health and safety of site workers. <br />Personnel exiting the site shall gather at the designated assembly point. To determine if <br />everyone has successfully exited the site, personnel will be accounted for at the assembly <br />point by the SSHO, PM, or other site official. If any worker is unable to be accounted for, <br />notification will be given to the SSHO, and appropriate action initiated. <br />Contractors and subcontractors on the site have coordinated their emergency response <br />plans to ensure that these plans are compatible and source(s) of potential emergencies are <br />recognized, alarm systems are clearly understood, and evacuation routes are accessible to <br />all site workers. <br />10.7 Emergency Decontamination Procedures <br />Emergency decontamination procedures shall be conducted by first responders. <br />10.8 Emergency Medical Treatment and First Aid <br />Any site worker or a visitor who requires medical care and/or is transferred to a medical <br />facility shall be accompanied by the SSHO, PM, or other site personnel to apprise medical <br />professionals. The closest Emergency Medical Care facility for the Site is: <br />Oak Valley Hospital <br />350 S Oak Ave., Oakdale, CA 95361 <br />The route to the Oak Valley Hospital is shown on Figure 2. <br />10.9 Emergency Alerting and Response Procedures <br />In an emergency situation, site workers and/or personnel shall notify the SSHO, or <br />designated alternate, who shall evaluate available information and initiate appropriate <br />response actions. Personnel at this site are notified of emergencies by use of an employee <br />alarm system. <br />The site uses the following type of communication devices and initiating the employee alarm <br />system: <br />• Contractors mobile cell phones <br />10.10 Critique of Response and Follow-up <br />After every emergency incident or evacuation from the site, the contractors SSHO, will <br />evaluate the quality and safety of response actions. Any deficiencies in response actions will <br />be included in a specific follow-up report that shall include any recommendations for
