HSC 117960- LQG shall file MWMP with the EA. <br /> OBSERVATIONS: <br /> The Medical Waste Management Plan was not available for review. <br /> CORRECTIVE ACTION: <br /> The Medical Waste Management Plan shall be available on site for review by the EHD. Operator is to update the Medical Waste <br /> Management Plan and provide a copy to the EHD. <br /> 13. HSC 117975- LQG shall maintain treatment and tracking documents for 2 years. <br /> OBSERVATIONS: <br /> The tracking documents indicate the sharps waste is disposed as biohazardous waste. EHD is unable to distinguish when the <br /> sharps waste is picked up by the medical waste hauler. <br /> CORRECTIVE ACTION: <br /> The sharps waste and biohazardous waste shall be disposed appropriately and documentation shall indicate when each type of was <br /> picked up by the hauler. <br /> 24. HSC 118275 - Medical, biohazardous, sharps,trace chemo, pathology, or pharmaceutical waste shall be segregated <br /> and contained pursuant to this section. The container shall be properly labeled. <br /> OBSERVATIONS: <br /> 1. A glove was observed on top of the chemotherapy waste container located under the counter of Suite C (see PHOTO 2). <br /> 2. A tight fitting lid was not observed for the chemotherapy waste container located under the counter of Suite C (see PHOTO 2) <br /> and the chemotherapy waste container located in the"Infusion Room" (see PHOTO 6). <br /> 3. The pharmaceutical waste container in the "Mixing Room" (see PHOTO 5) and in the"Infusion Room" (see PHOTO 6)were <br /> not properly labeled. <br /> CORRECTIVE ACTION: <br /> 1. Chemotherapy waste shall be contained inside of the chemotherapy waste container. <br /> 2. The chemotherapy waste container shall have a tight fitting lid. Provide photo graphic evidence of correction to the EHD. <br /> 3. Nonradioactive and non-RCRA pharmaceutical wastes, that are regulated as medical waste, shall be placed in a container or <br /> secondary container labeled with the words"HIGH HEAT OR INCINERATION ONLY" on the lid and sides, so as to be visible <br /> from any lateral direction. Provide photographic evidence of correction to the EHD. <br /> 25. HSC 118280 - Biohazard bags shall be containerized and stored pursuant to this section. <br /> OBSERVATIONS: <br /> The biohazard container in the"Infusion Room" (see PHOTO 6)was not properly labeled. <br /> CORRECTIVE ACTION: <br /> Biohazardous waste shall be bagged and placed in a rigid container which is leak resistant, have tight-fitting cover, and be kept <br /> clean and in good repair. Container shall be labeled with the words"Biohazardous Waste" or"BIOHAZARD" on the lid and on the <br /> sides so as to be visible from any lateral direction.
