Nonradioactive and non-RCRA pharmaceutical wastes, that are regulated as medical waste, shall be placed in a container or <br /> secondary container labeled with the words "HIGH HEAT OR INCINERATION ONLY'on the lid and sides, so as to be visible <br /> from any lateral direction. Ensure all pharmaceutical waste containers are equipped with functioning, closeable lids. <br /> 2. Nonradioactive and non-RCRA pharmaceutical wastes, that are regulated as medical waste, shall be placed in a container or <br /> secondary container labeled with the words"HIGH HEAT OR INCINERATION ONLY' on the lid and sides, so as to be visible <br /> from any lateral direction. Ensure all pharmaceutical waste containers are properly labeled. <br /> 3. Biohazardous waste shall be placed in a biohazard bag conspicuously labeled with words"Biohazardous Waste" or <br /> "BIOHAZARD". Ensure all biohazard containers are lined with red biohazard liners prior to using for disposal. <br /> 4. Trace chemotherapy waste shall be segregated for storage, and the secondary container shall be labeled with the words <br /> "Chemotherapy Waste" or"CHEMO" on the lid and sides, so as to be visible from any lateral direction. Ensure the chemotherapy <br /> waste containers are labeled properly. <br /> 25. HSC 118280 -Biohazard bags shall be containerized and stored pursuant to this section. <br /> OBSERVATIONS: <br /> 1. Biohazardous waste is stored above 32 F onsite for over 7 days. Records indicated biohazardous waste was not treated for <br /> more than 7 days on the following dates: <br /> Chamber 1 Chamber 2 <br /> 2/15/23 to 2/24/23 2/15/23 to 2/24/23 <br /> 9/12/23 to 9/28/23 9/12/23 to 9/28/23 <br /> 5/22/24 to 6/02/24 5/22/24 to 5/30/24 <br /> 6/3/24 to 6/11/24 6/26/24 to 7/5/24 <br /> 6/26/24 to 7/6124 7/5/24 to 7/25/24 <br /> 7/6/24 to 7/25/24 <br /> 2. Biohazardous waste containers located in Specimen Collection—Janitor Closet(see PHOTO 1), the Dialysis Clinic (see <br /> PHOTO 2), Medical and Dental Clinic—Soiled Utility Room (see PHOTO 3), and D7-HUT(see PHOTO 4)were not closed with a <br /> tight fitting lid.—Corrected on site <br /> 3.A RCRA container, located in the designated accumulation area, was stored inside of a biohazard bag with biohazard labels <br /> attached to the outside of the bag (see PHOTO 15).—Corrected on site. <br /> 4. A biohazard bag with waste observed on a rack between two biohazard containers near the autoclaves (see PHOTO 13). <br /> CORRECTIVE ACTIONS: <br /> 1. A person who generates 20 or more pounds of biohazardous waste per month shall not contain or store that waste above 32 F <br /> onsite for more than 7 days. Ensure biohazardous waste is not contained or stored for more than 7 days on site. If there is no <br /> biohazardous waste to autoclave then ensure it is documented. <br /> 2. Biohazardous waste shall be bagged and placed in a rigid container which is leak resistant, have tight-fitting cover, and be kept <br /> clean and in good repair. Container shall be labeled with the words "Biohazardous Waste" or"BIOHAZARD"on the lid and on the <br /> sides so as to be visible from any lateral direction. Ensure the biohazardous waste containers are closed and have a tight fitting <br /> lid. <br /> 3. Biohazardous waste shall be bagged and placed in a rigid container which is leak resistant, have tight-fitting cover, and be kept <br /> clean and in good repair. Biohazard bags must only be used for the storage of biohazardous waste. Biohazardous waste must be <br /> placed in a biohazard bag inside of a rigid container equipped with a tight fitting lid. Ensure biohazard bags are being used <br /> appropriately. <br /> 4. Biohazardous waste shall be bagged and placed in a rigid container which is leak resistant, have tight-fitting cover, and be kept <br /> clean and in good repair. Ensure biohazardous waste is always placed in a biohazard container. <br /> 29. HSC 118307-Intermediate storage area shall be secured and marked with proper signage. <br /> OBSERVATIONS: <br /> The biohazard sign outside of the MUT-AB building was faded (see PHOTO 9). <br /> Reinspecion on/about: A reinspection fee of$172 per hour may be charged.
