HSC 117967—Operator training. <br /> OBSERVATIONS: <br /> 1. Training records for autoclave operators were not provided. <br /> 2. Employees were not aware the autoclave bag has a heat sensitive tape (see PHOTO 11). <br /> CORRECTIVE ACTIONS: <br /> 1. Autoclave operators must be trained upon hire and annually thereafter. Records of training must be maintained on site for 2 <br /> years. Provide evidence of training to the EHD for the last 2 years. <br /> 2. Autoclave operators must be trained upon hire and annually thereafter. Train autoclave operators on the procedures used for <br /> the autoclave and provide record to the EHD. <br /> Correct all violations prior to the reinspection on September 25, 2024 <br /> NOTES: <br /> 1. Inspection report emailed to the following operators: Nikolas Corren (Nikolas.Correnacdcr.ca.4ov), Patrick Brown <br /> (Pat.Brownacdcr.ca.gov), Kevin Hendrick (Kevin.Hendrickacdcr.ca.gov) and Mark Whisler(Mark.Whislera-cdcr.ca.gov). <br /> 2. Conflicting information was provided to the EHD when operators were questioned about the sharps waste. After the inspection, <br /> Gelix Vicuna, provided information on proper treatment for the sharps waste via email. Sharps waste is sterilized on site. <br /> 3. The secondary biohazardous waste containers are lined with autoclave bags and then with biohazardous red bags. <br /> 4. The autoclave bags have indicator strips, which changes to brown after sterilization is complete. <br /> 5. The biohazardous red bags are ASTM D1933 and ASTM D1709 certified. <br /> 6. The following areas were inspected: Pharmacy, Imaging/Diagnostics, Specimen Collection, Standby Emergency, Dialysis, <br /> Medical/Dental Clinic, Yard B (building 61), Yard C (building C6), Yard D (D1 and D7) and the warehouse. <br /> 7. Yellow bags containing contaminated linen observed inside of a biohazard container located in the proximity of the autoclaves. <br /> Per operator, the yellow bags were accidentally transported to the sterilization. Yellow bags with linen are transported off site. <br /> 8. The lids of the biohazardous waste containers require force to completely close and cover the containers. The lids are <br /> deformed from heat while cleaning. <br /> Reinspecion on/about: A reinspection fee of$172 per hour may be charged. Page 4 of 4 <br /> EH-2/2023 <br />
