Nonradioactive and non-RCRA pharmaceutical wastes, that are regulated as medical waste, shall be placed in a container or <br /> secondary container labeled with the words"HIGH HEAT OR INCINERATION ONLY" on the lid and sides, so as to be visible <br /> from any lateral direction. <br /> 4. Sharps waste shall be contained in a United State Food and Drug Administration approved sharps container. Operator shall <br /> replace the 5-gallon bucket with an approved container and provide photographic evidence to the EHD. <br /> 5. Medical waste shall be contained separately from other waste at the point of origin. Train staff on solid waste/medical waste <br /> segregation and provide training record to the EHD. <br /> 25. HSC 118280 -Biohazard bags shall be containerized and stored pursuant to this section. <br /> OBSERVATIONS: <br /> 1. The biohazard waste containers located in the Soiled Utility Room of the Standby ER (see PHOTO 2—CORRECTED), in the <br /> Dialysis Clinic (see PHOTO 3 and PHOTO 5), in the Soiled Utility Room of Al B (see PHOTO 10) and in the Soiled Utility Room <br /> of B1A(see PHOTO 12—CORRECTED)were not covered with a tight fitting lid. <br /> 2. The biohazard label is peeling from the biohazard container located in the Soiled Utility Room of A4 (see PHOTO 9). <br /> 3. Records indicated biohazardous waste was not treated for more than 7 days from October 27, 2024 to November 13, 2024 <br /> and again from March 4, 2025 to March 25, 2025. <br /> CORRECTIVE ACTIONS: <br /> 1. Biohazardous waste shall be bagged and placed in a rigid container which is leak resistant, have tight fitting cover, and be <br /> kept clean and in good repair. Container shall be labeled with the words"Biohazardous Waste"or"BIOHAZARD" on the lid and <br /> on the sides so as to be visible from any lateral direction. Ensure the biohazard waste containers are closed with a tight-fitting <br /> lid. Provide photographic evidence of correction to the EHD. <br /> 2. Biohazardous waste shall be bagged and placed in a rigid container which is leak resistant, have tight fitting cover, and be <br /> kept clean and in good repair. Container shall be labeled with the words"Biohazardous Waste" or"BIOHAZARD" on the lid and <br /> on the sides so as to be visible from any lateral direction. Replace peeling labels and provide photographic evidence of <br /> correction to the EHD. <br /> 3.A person who generates 20 or more pounds of biohazardous waste per month shall not contain or store that waste above 32 <br /> F onsite for more than 7 days. Ensure biohazardous waste is either treated on site or hauled off site. If there is no biohazard <br /> waste, then ensure it is documented. <br /> 26. HSC 118285-Sharps waste shall be containerized and stored pursuant to this section. <br /> OBSERVATIONS: <br /> The sharps waste containers were not closed to prevent spillage in the Dialysis Clinic (see PHOTO 4 and PHOTO 5). <br /> CORRECTIVE ACTIONS: <br /> Sharps waste shall be placed in a sharps container. Full sharps container ready for disposal shall be taped closed or tightly lid <br /> and stored onsite not more than 30 days. Sharps container shall be labeled with the words"Sharps Waste" or BIOHAZARD". <br /> Operator shall replace sharps containers that immediately lock when lids are closed. Provide EHD photographic evidence of <br /> replacement containers. <br /> 31. HSC 117750—Sharps container <br /> OBSERVATIONS: <br /> The sharps waste container located at the Nurses Station of D1A was lined. <br /> CORRECTIVE ACTIONS: <br /> Sharps containers shall not be lined with a plastic bag or inner liner. Ensure the sharps waste container is not lined. <br /> Correct all violations and provide evidence of corrections to the EHD <br /> by September 24, 2025. <br /> Reinspecion on/about: A reinspection fee of$179 per hour may be charged.
