Environmental Health Department <br />CP22 - HMBP Inspection Report <br />Date: <br />September 24, 2024 <br /> Facility Address: <br /> 3837 Imperial WAY, Stockton <br /> Facility Name: <br /> MITSUBISHI CHEMICALS ADVANCED <br />SUMMARY OF VIOLATIONS <br />(CLASS I, CLASS II, or MINOR - Notice to Comply) <br />RemarksItem # <br />(2) For the purpose of this subdivision, “substantial change” means any change in a facility that would inhibit <br />immediate response during an emergency by either site personnel or emergency response personnel, or that could <br />inhibit the handler's ability to comply with Section 25507 , change the operational knowledge of the facility, or impede <br />implementation of the business plan. <br />This is (Class 2) Violation. <br />CA Health and Safety Code 25508(a)(1) 1010003 - Business Owner/Operator ID and Business Activities pages <br />electronically submitted and are accurate. <br />OBSERVATION: The Business Owner/Operator Identification form was not submitted for 2024. The 2023 submittal has <br />an inaccurate EPA ID number, as the current EPA ID number is CAL 00452598 <br />REGULATION GUIDANCE: This information must be current and submitted as part of the business plan. <br />CORRECTIVE ACTION: Immediately log into the California Environmental Reporting System (CERS) at <br />http://cers.calepa.ca.gov/, enter the correct or updated information, and submit to the EHD for approval. <br />This is (Minor) Violation. <br /> 9 <br />CA Health and Safety Code 25505(a)(1) 1010004 - Complete and accurate Hazardous Materials Inventory information <br />electronically submitted. <br />OBSERVATION: The following hazardous materials were observed on-site during the inspection, but were not listed in <br />the hazardous materials inventory: <br />~165 gallons of Propylene Glycol <br />~55 gallons of Oemeta Coolant <br />~110 gallons of Oily Solids (waste) <br />REGULATION GUIDANCE: Any material that meets or exceeds the reportable quantity shall be reported in the <br />facility's business plan. <br />CORRECTIVE ACTIONS: Immediately log into the California Environmental Reporting System (CERS) at <br />http://cers.calepa.ca.gov/, add the materials to the hazardous material inventory, and submit to the EHD for approval. <br />This is (Minor) Violation. <br /> 10 <br />CA Health and Safety Code 25505(a)(2) 1010005 - Site Map with all required content electronically submitted. <br />OBSERVATION: The site map is incomplete as it is missing the following elements: <br />-North Facing Arrow <br />-Location of Hazardous Materials <br />REGULATION GUIDANCE: The site map is required to contain an indicator of north orientation, adjacent streets, <br />access and exit points, evacuation staging areas, hazardous material handling and storage areas, emergency <br />response equipment, and, if present, loading areas, internal roads, storm and sewer drains, and emergency shutoffs , <br />as well as additional map requirements the governing body of the unified program agency finds necessary. <br /> 11 <br />FA0019701 PR0529864 SC001 09/24/2024 <br />EHD Rev. 12/06/2021 CP22 - HMBP OIRPage 3 of 5 <br />1868 E.
