Reports from June 2022 through December 2022 <br /> were not present on site. <br /> REGULATION GUIDANCE: Designated operator inspection reports prior to October 1, 2018 shall be retained on site for <br /> twelve months. Designated operator inspection reports on or after October 1, 2018 shall be retained on site for thirty-six <br /> months. <br /> CORRECTIVE ACTION: Locate and ensure that copies of the previous thirty-six months of designated operator <br /> inspection reports are maintained on site. Submit copies of the missing reports to the EHD. <br /> This is a (Minor)Violation. <br /> 56 CA Code of Regulations 2716(a-e) 2030013 - DO inspections completed at least every 30 days and meet all <br /> requirements (TCR 12) <br /> OBSERVATION 1: Designated operator reports were not signed within the required time frame. <br /> -Multiple designated operator reports were not signed and dated by the owner/operator <br /> REGULATORY GUIDANCE: Within 48 hours of the completion of the designated UST operator visual inspection, the <br /> designated operator shall sign and provide the owner or operator with a copy of the inspection report. Within 72 hours of <br /> being provided a signed copy of the report, the owner or operator shall provide a description of each corrective action <br /> taken or to be taken and shall sign and date the report, acknowledging the identified compliance issues. <br /> CORRECTIVE ACTION: Ensure all designated operator inspection reports are completed and signed within the required <br /> time frame in the future. <br /> OBSERVATION 2: The designated operator failed to check and note all required items on the visual inspection reports. <br /> The incorrect and missing information included the following: <br /> -Multiple reports only list"BZ Service Station Maintenance, Inc" but do not list the DO name in section 2 <br /> -The DO failed to record the date the report was provided to the owner in section 4 of the 10/2/24 report <br /> REGULATION GUIDANCE: During the visual inspection, the designated operator shall check that all required testing and <br /> maintenance for the UST system have been completed and shall check all required items on the report. <br /> CORRECTIVE ACTION: Ensure that designated operators performing visual inspections at this facility are including all of <br /> the required information on the reports. <br /> This is a Repeat(Class 2)Violation. <br /> EHD-CUPA Inspection Report IS02 Page 5 of 7 FA0000212 PR0231692 4519949 6/25/2025 <br /> 1868 E.
