SAN J OAQ U N Environmental Health Department <br /> Underground Storage Tank Program Inspection Report <br /> Facility Name: Facility Address: Date: <br /> Mossdale Chevron 1444 W MOSSDALE RD, LATHROP June 25, 2025 <br /> 57 CA Code of Regulations 2715(c) 2030012 - Initial and annual training provided to employees and at least one trained <br /> employee is present <br /> OBSERVATION: The designated operator employee training for all employees was performed late. The 2023 training <br /> date is listed as 6/2/2023 and the 2024 training date is listed as 7/9/24, one month late. <br /> REGULATION GUIDANCE: The designated operator shall train facility employees in the proper operation and <br /> maintenance of the UST system at least once every 12 months. The training shall be conducted before the individual <br /> performs the duties of a facility employees. The training shall include, but is not limited to: <br /> 1. Operation of the UST system in a manner consistent with the facility's best management practices <br /> 2. Employee's role with regard to monitoring equipment as specified in the facility's monitoring plan <br /> 3. Employee's role with regard to spills and overfills as specified in the facility's response plan <br /> 4. Name of the contact person(s)for emergencies and monitoring equipment alarms Initial training shall also include a <br /> site-specific practical demonstration that provides both sharing information and evaluating knowledge for responding to <br /> emergencies, leak detection, monitoring alarms, spills and overfills. <br /> At least one of the facility employees present during operating hours shall have current training. <br /> CORRECTIVE ACTION: Ensure that employees have been trained by the designated operator, maintain the list on site, <br /> and submit a copy of the training records to the EHD. <br /> This is a Repeat(Class 2)Violation. <br /> 58 CA Code of Regulations 2715(c)(3), 2715(c)(4) 2010015- Copies of Facility Employee Training Certificates maintained <br /> OBSERVATION: The Facility Employee Training Certificates dated 6/2/23 and 7/9/24 are inaccurate and incomplete. The <br /> initial training dates for the same employees are different on the 2023 and the 2024 form and they do not list the dates of <br /> assuming responsibility for each employee. <br /> REGULATION GUIDANCE: All site employees must be trained prior to taking responsibility as a facility employee if after <br /> October 1, 2018, or within 30 days of hire if prior to October 1, 2018. The Facility Employee Training Certificate shall <br /> contain accurate information for all employees trained. <br /> CORRECTIVE ACTION: Update the Facility Employee Training Certificate to contain the accurate information and <br /> submit a copy of the updated form to the EHD. <br /> This is a Repeat(Class 2)Violation. <br /> 77 CA Code of Regulations 2712(f) 2030073- Facility has corrected cited violations within 30 days of receiving report <br /> OBSERVATION: An inspection was last done on 6/18/24 and an inspection report was issued identifying information to <br /> be submitted to bring this site into compliance. This information was required to be submitted by 8/1/24. This information <br /> has not been received resulting in a non-compliant status for this facility. <br /> REGULATION GUIDANCE: An operator that receives an inspection report shall have 30 days to submit a written <br /> response that includes a statement documenting corrective actions taken or proposing corrective actions which will be <br /> taken. <br /> CORRECTIVE ACTION: Ensure that a written response documenting corrective actions taken or proposed is submitted <br /> within 30 days of receiving an inspection report. <br /> This is a (Minor)Violation. <br /> EHD-CUPA Inspection Report IS02 Page 6 of 7 FA0000212 PR0231692 4519949 6/25/2025 <br /> 1868 E.
