OBSERVATION:The designated operator failed to check and note all required items on the visual inspection reports. The incorrect and missing information included the following from the inspection reports: -The name of the designated operator in section 2 lists"BZ Service Station Maintenance, Inc"but does not list the name of the designated operator on the 2/21/2025 DO report. REGULATION GUIDANCE: During the visual inspection,the designated operator shall check that all required testing and maintenance for the UST system have been completed and shall check all required items on the report. CORRECTIVE ACTION: Ensure that designated operators performing visual inspections at this facility are including all of the required information on the reports. OBSERVATION:The facility owner/operator failed to check and note all required items on the visual inspection reports. The incorrect and missing information included the following from the inspection reports: -On the 2/21/25 DO report, the DO documented the L15 low liquid alarm and stated that the"site will call in" regarding the alarm. Store Manager Inder confirmed they had planned to call in the alarm to request maintenance but stated that the alarm cleared itself before they were able to call. This was not documented, section 5 of the report was left blank. REGULATION GUIDANCE: During the visual inspection,the designated operator shall check that all required testing and maintenance for the UST system have been completed and shall check all required items on the report. CORRECTIVE ACTION: Ensure that designated operators performing visual inspections at this facility are including all of the required information on the reports. This is a repeat violation (Class 2) Violation. 59 CA Code of Regulations 2712(b)(1-2)2030001 - Records of release detection monitoring and maintenance properly maintained (TCR 9d). OBSERVATION: Maintenance and monitoring records for the last three years were not found on site. -Overfill prevention test report for the diesel tank done 5/10/24 -Overfill prevention test report for the 87 and 91 tanks done 5/16/24 REGULATION GUIDANCE:These records shall be maintained on site for at least three years. Monitoring records include: (1)date and time of all monitoring or sampling; (2) monitoring equipment calibration and maintenance records; (3)results of any visual observations; (4)results of sample analysis performed a lab or in the field; (5)logs of all readings of gauges or other monitoring equipment, ground water elevations, or other test results; (6)results of inventory readings and reconciliations. CORRECTIVE ACTION: Immediately locate and maintain all missing maintenance and monitoring records for the last three years on site. Submit copies of any missing work orders and alarm records to the EHD. This is (Minor)Violation. 72 CA Code of Regulations 2712 2030039-Compliance with all operating permit requirements.
