The most recent financial responsibility documents in CERS are dated 1/10/2024. REGULATION GUIDANCE: Current financial responsibility documents are required to be submitted annually. CORRECTIVE ACTION: Immediately log into the California Environmental Reporting System (CERS)at http://cers.calepa.ca.gov/, and upload the required documents. This is a repeat violation (Class 2) Violation. 56 CA Code of Regulations 2716(a-e)2030013-DO inspections completed at least every 30 days and meet all requirements(TCR 12). OBSERVATION:The designated operator failed to check and note the correct test dates for the overfill prevention equipment on the designated operator inspection reports. The correct date for overfill prevention equipment was 1/11/2022.The routine overfill prevention equipment testing was performed at the time of inspection, on the date of 1/13/2025. REGULATION GUIDANCE: During the inspection, the designated operator shall check that all required testing and maintenance for the UST system have been completed and document the dates they were done. CORRECTIVE ACTION: Ensure that designated operators performing inspections at this facility are including all of the required information on the reports. This is a repeat violation (Class 2) Violation. 57 CA Code of Regulations 2715(c)2030012-Initial and annual training provided to employees and at least one trained employee is present. OBSERVATION:The designated operator employee training for Jas Bains was not current.The"Facility Employee Training Certificate," located in Appendix XI of California Code Of Regulations, Title 23, Division 3, Chapter 16 was not found for the past year. Jas stated that the designated operator performed this training within the past year. REGULATION GUIDANCE:The designated operator shall train facility employees in the proper operation and maintenance of the UST system at least once every 12 months. The training shall be conducted before the individual performs the duties of a facility employees.The training shall include, but is not limited to: 1. Operation of the UST system in a manner consistent with the facility's best management practices 2. Employee's role with regard to monitoring equipment as specified in the facility's monitoring plan 3. Employee's role with regard to spills and overfills as specified in the facility's response plan 4. Name of the contact person(s)for emergencies and monitoring equipment alarms Initial training shall also include a site-specific practical demonstration that provides both sharing information and evaluating knowledge for responding to emergencies, leak detection, monitoring alarms, spills and overfills. At least one of the facility employees present during operating hours shall have current training. 23 CCR§2715(c)(4). "...upon completion of the facility employee training, the designated UST operator(s)shall provide a signed "Facility Employee Training Certificate,"located in Appendix XII..." CORRECTIVE ACTION: Ensure that employees have been trained by the designated operator, maintain the list on site, and submit a copy of the training records to the EHD.
