Immediately contact a properly licensed, trained, and certified contractor <br /> to address the water intrusion into the diesel STP, 91 STP, and 87 STP sumps under permit and inspection of the <br /> EHD, if needed. <br /> This is a repeat violation (Class 2) Violation. <br /> 53 CA Health and Safety Code 25292.2; CCR 2711(a)(11)2010007-Submitted/maintained current Cert of Financial <br /> Responsibility/mech of financial assurance(TCR 11). <br /> OBSERVATION: Current financial responsibility documents were not found on site. <br /> REGULATION GUIDANCE:A copy of current financial responsibility documents are required to be maintained on site. <br /> CORRECTIVE ACTION: Immediately obtain a copy of the facility's financial responsibility documents and maintain a <br /> copy on site. <br /> This is a repeat violation (Class 2) Violation. <br /> 56 CA Code of Regulations 2716(a-e)2030013-DO inspections completed at least every 30 days and meet all <br /> requirements(TCR 12). <br /> OBSERVATION:The designated operator failed to document all the alarms from the attached alarm history on the <br /> 6/24/25, 2/4/25,12/10/24, 11/12/24 designated operator inspection report and failed to check that they were responded <br /> to appropriately. The missing alarms include: Q4 alarms, brine sensor <br /> REGULATION GUIDANCE: During the inspection,the designated operator shall review the alarm history since the <br /> previous inspection, check that each alarm was documented and responded to appropriately, and attach a copy of the <br /> alarm history with documentation taken in response to any alarms to the inspection report. <br /> CORRECTIVE ACTION: Ensure that designated operators performing inspections at this facility are including all of the <br /> required information on the reports. <br /> This is a repeat violation (Class 2) Violation. <br /> 72 CA Code of Regulations 2712 2030039-Compliance with all operating permit requirements. <br /> OBSERVATION:The UDC 1-2 sensors was repaired or replaced and a permit was not obtained from the EHD. Per <br /> the DO notes,the sensor alarms have been on in UDC 1-2 since the it had caught fire. <br /> REGULATION GUIDANCE: Emergency repairs relating to UST system monitoring and release detection may be <br /> performed by a properly licensed,trained, and certified contractor pursuant to the EHD's policy and a permit obtained <br /> within one business day from the EHD. All other repairs must have a permit submitted to and approved by the EHD <br /> prior to beginning work. <br /> CORRECTIVE ACTION: Immediately obtain a permit from the EHD for the UDC 1-2 sensors alarms.
