A dOAQU1N Environmental Health Department <br /> CQLJIVTY AMENDED <br /> 1/27/2025 <br /> CP22 - UST FULL Inspection Report <br /> Facility Name: Facility Address: Date: <br /> KWIK SERV LODI BW 113* 420 W KETTLEMAN LN, LODI January 24 2025 <br /> SUMMARY OF VIOLATIONS <br /> (CLASS I,CLASS II,or MINOR-Notice to Comply) <br /> Item# Remarks <br /> REGULATION GUIDANCE: Since the independent testing organization for the Veeder Root 343 annular sensor and <br /> Veeder Root 208 piping sensor did not include a statement of compatibility for E85 to be used in the UST system, a <br /> written, affirmative statement of compatibility for the E85 from the manufacturer shall be submitted. <br /> CORRECTIVE ACTION: Immediately submit an affirmative statement of compatibility or replace the Veeder Root 343 <br /> annular sensor and Veeder Root 208 piping sensor with a component that meets all applicable Title 23 requirements <br /> for a UST system. <br /> This is a repeat violation (Class 2) Violation. <br /> 72 CA Code of Regulations 2712 2030039-Compliance with all operating permit requirements. <br /> OBSERVATION:The Diesel 208 piping senor was replaced on December 11, 2023 and a permit was not obtained from <br /> the EHD.A permit SR0087753 was applied for but has not been approved.Additional information was requested on <br /> May 2, 2024 and has not yet been received. <br /> REGULATION GUIDANCE: Emergency repairs relating to UST system monitoring and release detection may be <br /> performed by a properly licensed,trained, and certified contractor pursuant to the EHD's policy and a permit obtained <br /> within one business day from the EHD. All other repairs must have a permit submitted to and approved by the EHD <br /> prior to beginning work. <br /> CORRECTIVE ACTION: Immediately obtain an approved permit from the EHD for the Diesel 208 piping sensor <br /> replacement on December 11, 2023. Submit missing documentation to have an approved permit for SR0087753 or <br /> apply for a new permit. A"Failure to Obtain a Permit"penalty applies. <br /> This is a repeat violation (Class 2) Violation. <br /> 77 CA Code of Regulations 2712(f)2030073- Facility has corrected cited violations within 30 days of receiving report. <br /> OBSERVATION:An inspection was last done on January 11, 2024 and an inspection report was issued identifying <br /> information to be submitted to bring this site into compliance. This information was required to be submitted by <br /> February 10, 2024.This information was received on June 5, 2024, 116 days late(CERS submittal was made with <br /> some corrections on February 14, 2024-4 days late). <br /> REGULATION GUIDANCE:An operator that receives an inspection report shall have 30 days to implement the <br /> corrections specified in the inspection report and comply with permit conditions. The corrective action shall include all <br /> of the recommendations made in the report. The operator shall submit a written response that includes a statement <br /> documenting corrective actions taken or proposing corrective actions which will be taken. <br /> CORRECTIVE ACTION: Ensure that a written response documenting corrective actions taken or proposed is <br /> submitted and all corrections implemented within 30 days of receiving an inspection report. <br /> This is a repeat violation (Class 2) Violation. <br /> Overall Inspection Comments: <br /> Routine UST Inspection <br /> An inspection checklist was provided to the facility operator on the day of inspection.
