Environmental Health Department <br />CP22 - AST Non-Qualified Inspection Report <br />Date: <br />December 04, 2024 <br /> Facility Address: <br /> 14900 W HWY 12 , LODI <br /> Facility Name: <br /> Yogi Bear's Jellystone Park <br />SUMMARY OF VIOLATIONS <br />(CLASS I, CLASS II, or MINOR - Notice to Comply) <br />RemarksItem # <br />exempted from the requirements of this part under § 112.1(d)(11). <br />CORRECTIVE ACTION: Ensure that the SPCC plan includes a complete facility diagram. Provide proof of correction to <br />the EHD. <br />This is (Minor) Violation. <br />Code of Fed Regulations 112.7(e), 112.8(c)(6) 4030015 - Aboveground containers tested or inspected for integrity <br />based on industry standards. <br />OBSERVATION: Facility failed to test & inspect each aboveground container for integrity based on industry standards, <br />on a regular schedule as discussed in the SPCC Plan. <br />- Section 5.2.8 of SPCC plan mentions that the overfill prevention system must be tested monthly and yearly. Despite <br />this, the inspection checklist that is completed monthly and yearly only has 2 questions addressing overfill and on <br />multiple inspections the inspector responds with "N/A". Additionally, Patrick Glover was asked how the overfill <br />prevention system is tested and an answer could not be provided. <br />- Table 5-2.6: Scope and Frequency of Bulk Storage Containers Inspections and Tests indicates that an inspection by <br />a certified inspector (as per API 570) should be done annually. As of December 4, 2024 the last inspection was done <br />on April 7, 2023 which exceeds the annual requirement. <br />The Pressure Test (Pipe secondary) was also last done on April 7, 2023 which exceeds the annual requirement. <br />The table mentioned above also shows that a vacuum test (tank annular), Pressure Test (Pipe secondary) and <br />Hydrostatic Test (UDC) is to be done annually. <br />Note: At time of inspection records for a vacuum test (tank annular) and Hydrostatic Test (UDC) were not found and <br />not discussed with Patrick Glover, this does not negate the requirement set in place by the engineer that approved the <br />SPCC plan, refer to SPCC plan for guidance. <br />REGULATION GUIDANCE: REGULATION GUIDANCE: 112.8(c)(6) Test or inspect each aboveground container for <br />integrity on a regular schedule and whenever you make material repairs. <br />CORRECTIVE ACTION: Immediately conduct the necessary integrity testing on the aboveground storage tanks as <br />mentioned in the SPCC plan based on industry standards on a regular schedule. Maintain testing and inspection <br />records. Submit a copy of the test/ inspection results to the EHD. <br />This is (Class 2) Violation. <br /> 56 <br />Code of Fed Regulations 112.7(e), 112.8(c)(6) 4010021 - Written Inspection/testing procedures kept, inspection/testing <br />records signed, kept for 3 years. <br />OBSERVATION: The facility failed to keep written procedures and records of inspections and tests, including integrity <br />tests, for at least three years. <br />- The monthly inspection was not found for January 2024 <br />REGULATION GUIDANCE: 112.7(e), Conduct inspections and tests required by this part in accordance with written <br /> 57 <br />FA0000527 PR0516677 SC001 12/04/2024 <br />EHD Rev. 12/06/2021 CP22 - AST Non-Qualified OIRPage 7 of 9 <br />1868 E.
