S N J VAQ V I N Environmental Health Department <br /> x <br /> COUNT <br /> � + reorness grows here. <br /> COMPLAINT INSPECTION REPORT <br /> Name of Facility: BURGER KING#004886 Date: 02/11/2025 <br /> Address: 4571 N PERSHING , STOCKTON 95207 <br /> Owner/Operator: GAI MANAGEMENT Telephone: (925)292-8359 <br /> Program Element: 1600- Complaint#: CO2500099 <br /> Responsible Party Contact: <br /> DESCRIPTION OF COMPLAINT ALLEGATIONS <br /> The San Joaquin County Environmental Health Department received the following complaint: <br /> Complainant states they ate value onion rings at 4:30PM on 02-10.At 2AM they became sick with vomiting. Complainant <br /> states they had a bad taste in their mouth and throat all day and had a hard time breathing as well as inflamed throat. <br /> Complainant also states they had a bad headache and feels horrible. <br /> OVERALL INSPECTION NOTES AND COMMENTS <br /> COMMENTS AND OBSERVATIONS <br /> Complaint inspection. No major violations. No re-inspection. <br /> Manager unaware of complaint and has not received any other complaints. No employees sick per manager. Onions rings are <br /> pre-made and delivered frozen. Onion rings are stored in the walk-in freezer, moved to the two door freezer as needed and <br /> cooked as ordered. Freezers were both in good working order. No evidence of contamination observed. Hood over fryers was <br /> clean at the time of the inspection. Onion ring out of the fryer was temped at 197 F. Hot water was available. Hand sink was <br /> complete with warm&cold water and soap&paper towels. Quat sanitizer buckets were set up.
