At least one of the facility employees present during operating hours shall have current training. <br /> CORRECTIVE ACTION: Ensure that employees have been trained by the designated operator, maintain the list on <br /> site,there is always a designated operator trained person on site and submit a copy of the training records to the <br /> EHD. <br /> This is (Class 2)Violation. <br /> 58 CA Code of Regulations 2715(c)(3), 2715(c)(4)2010015- Copies of Facility Employee Training Certificates maintained. <br /> OBSERVATION: The Facility Employee Training Certificate dated 10/18/2024 and 2/7/2025 was combined on one <br /> form.These should be documented on separate forms, one for each date of training. In addition, only the first names <br /> for Paula and Mohammad were listed. Full names should be listed. <br /> REGULATION GUIDANCE:All site employees must be trained prior to taking responsibility as a facility employee if <br /> after October 1, 2018, or within 30 days of hire if prior to October 1, 2018. The Facility Employee Training Certificate <br /> shall contain accurate information for all employees trained. <br /> CORRECTIVE ACTION: Update the Facility Employee Training Certificate to contain the accurate information and <br /> submit a copy of the updated form to the EHD. <br /> This is (Minor)Violation. <br /> 59 CA Code of Regulations 2712(b)(1-2)2030001 - Records of release detection monitoring and maintenance properly <br /> maintained (TCR 9d). <br /> OBSERVATION: Maintenance and monitoring records for the last three years were not found on site.The following <br /> were not found on site: <br /> -2/7/2023 monitoring system certification and spill container results <br /> -2/12/2024 monitoring system certification and spill container results <br /> REGULATION GUIDANCE:These records shall be maintained on site for at least three years. Monitoring records <br /> include: (1)date and time of all monitoring or sampling; (2)monitoring equipment calibration and maintenance records; <br /> (3)results of any visual observations; (4)results of sample analysis performed a lab or in the field; (5)logs of all <br /> readings of gauges or other monitoring equipment, ground water elevations, or other test results; (6)results of <br /> inventory readings and reconciliations. <br /> CORRECTIVE ACTION: Immediately locate and maintain all missing maintenance and monitoring records for the last <br /> three years on site.
