The financial responsibility <br /> documents observed during inspection were dated 1/15/2023. <br /> REGULATION GUIDANCE: A copy of current financial responsibility documents are required to be maintained on site. <br /> CORRECTIVE ACTION: Immediately obtain a copy of the facility's financial responsibility documents and maintain a <br /> copy on site. <br /> This is a Repeat(Class 2)Violation. <br /> 56 CA Code of Regulations 2716(a-e) 2030013 - DO inspections completed at least every 30 days and meet all <br /> requirements (TCR 12) <br /> OBSERVATION: The designated operator failed to check and note the correct test dates for the monitoring system <br /> certification and spill container testing on the designated operator inspection reports. The monitoring system and spill <br /> container testing were noted as 9/5/2023 for the 10/3,10/31 and 11/29 DO reports. The correct date for monitoring <br /> system and spill container testing is 9/17/24. <br /> REGULATION GUIDANCE: During the inspection, the designated operator shall check that all required testing and <br /> maintenance for the UST system have been completed and document the dates they were done. <br /> CORRECTIVE ACTION: Ensure that designated operators performing inspections at this facility are including all of the <br /> required information on the reports. <br /> This is a Repeat(Class 2)Violation. <br /> 59 CA Code of Regulations 2712(b)(1-2) 2030001 - Records of release detection monitoring and maintenance properly <br /> maintained (TCR 9d) <br /> OBSERVATION: Maintenance and monitoring records for the last three years were not found on site. The following test <br /> results were not observed: <br /> -2022 Monitoring system certification and spill container test results <br /> -2024 Overfill prevention equipment test results <br /> -2024 Secondary containment test results <br /> REGULATION GUIDANCE: These records shall be maintained on site for at least three years. Monitoring records <br /> include: (1)date and time of all monitoring or sampling; (2) monitoring equipment calibration and maintenance records; <br /> (3) results of any visual observations; (4) results of sample analysis performed a lab or in the field; (5) logs of all readings <br /> of gauges or other monitoring equipment, ground water elevations, or other test results; (6) results of inventory readings <br /> and reconciliations. <br /> CORRECTIVE ACTION: Immediately locate and maintain all missing maintenance and monitoring records for the last <br /> three years on site.
