Notifications Requirements . <br /> ❑ FRAs ONLY Make Initial Notifications <br /> — Plant (Evacuation to safe area upwind of the release but be sure there is a way to <br /> escape if required.) <br /> — Supervisor or Security and Maintenance <br /> — 911 If needed (Fire, Police, HazMat, and Ambulance as appropriate) <br /> ❑ Notify Incident Commanders for Plant refer to EAP/ERP for info. 1 <br /> ❑ Notify Corporate/Plant Upper Management > <br /> ❑ "Mandatory" Agency notifications for chemical releases < <br /> — Local 911 Local dispatch (Fire, Ambulance, Police, HazMat) [Plant or IC] <br /> — National Response Center (800) 424-8802 [IC or Plant Management] <br /> — State Warning Center ??? [IC or Plant Management] <br /> — CUPA/Administering Agency ??? [IC or Plant Management] <br /> — Others (e.g. Local Water Treatment Plant) if appropriate <br /> ❑ Refer to Facilities EAP Notification Section for phone #'s <br /> REMEMBER: IF 911 is called or the Fire Department responds for a chemical release the <br /> other Mandatory Agency Notifications (such as Nat'l Response Center, State, and County) <br /> must be made ASAP immediately within 15 minutes! <br />
