The Unified Program Agency (UPA) <br />phone number was not listed on emergency response and contingency plan. Add 209-468-3420 as the local UPA <br />phone number. <br />REGULATION GUIDANCE: A business plan shall include the following emergency response procedures for a <br />release or threatened release of hazardous materials, including, but not limited to, the following: - immediate <br />notification of local emergency personnel and the Environmental Health Department (EHD) - procedures for the <br />mitigation of a release or threatened release to minimize any potential harm or damage to persons, property, or the <br />environment - evacuation plans and procedures, including immediate notice, for the site CORRECTIVE ACTION: <br />Immediately log into the California Environmental Reporting System (CERS) at http://cers.calepa.ca.gov/, upload the <br />correct or updated information, and submit to the EHD for approval. <br />This is a repeat violation (Class 2) Violation. <br />14 <br />CA Health and Safety Code 25505(a)(4) 1020002 - Provided initial and annual training and maintained training records <br />for a minimum of three years. <br />OBSERVATION: Employee training records were unavailable for review at time of inspection. <br />REGULATION GUIDANCE: The business plan shall include provisions for ensuring that appropriate personnel <br />receive initial and annual refresher training. All employees shall be trained in safety procedures in the event of a <br />release or threatened release of a hazardous material, including, but not limited to, familiarity with the following plans <br />and procedures: (A) Immediate notification contacts to the appropriate local emergency response personnel and to <br />the unified program agency. (B) Procedures for the mitigation of a release or threatened release to minimize any <br />potential harm or damage to persons, property, or the environment. (C) Evacuation plans and procedures, including <br />immediate notice, for the business site. This training shall be documented electronically or by hard copy and shall <br />be made available for a minimum of three years. CORRECTIVE ACTION: Immediately provide employee training for <br />appropriate personnel and submit a copy of the training records to the EHD. <br />This is (Minor) Violation. <br />Overall Inspection Comments: <br />Conducted a Hazardous Materials Business Plan (HMBP) inspection. There were 5 violations found during the <br />inspection. Complete and submit a copy of the Return to Compliance Certification form to the EHD with a <br />statement documenting the corrective actions that have been or will be taken for each violation, and any <br />supporting paperwork, by 9/12/2024. <br />To minimize person to person contact EHD is choosing to write the name of person receiving the report instead of <br />having them sign.
