A dOAQU1N Environmental Health Department <br /> COUNTY <br /> CP22 - UST FULL Inspection Report <br /> Facility Name: Facility Address: Date: <br /> CAGASOLINE INC 2115 W YOSEMITE AVE, MANTECA March 20 2025 <br /> SUMMARY OF VIOLATIONS <br /> (CLASS I,CLASS II,or MINOR-Notice to Comply) <br /> Item# Remarks <br /> REGULATION GUIDANCE: During the inspection,the designated operator shall review the alarm history since the <br /> previous inspection, check that each alarm was documented and responded to appropriately, and attach a copy of the <br /> alarm history with documentation taken in response to any alarms to the inspection report. <br /> OBSERVATION:The designated operator failed to check and note the correct test dates for the overfill testing on the <br /> designated operator inspection reports: <br /> 4/30, 5/29, 6/26, 7/24, 8/21, 9/18, 10/16, 11/12, 12,11, 1/8, 2/5, and 3/6. <br /> The correct date for overfill testing is 3/25/2024. <br /> REGULATION GUIDANCE: During the inspection, the designated operator shall check that all required testing and <br /> maintenance for the UST system have been completed and document the dates they were done. <br /> CORRECTIVE ACTION: Ensure that designated operators performing inspections at this facility are including all of the <br /> required information on the reports. <br /> This is a repeat violation (Class 2) Violation. <br /> 67 CA Code of Regulations 2712(i)2030078-Owner/Operator retained a paper or electronic copy of the permit to operate <br /> at the facility. <br /> OBSERVATION:A copy of the UST operating permit was not found on site. <br /> REGULATION GUIDANCE:A copy of the UST operating permit and all attachments and conditions shall be retained at <br /> the facility. <br /> CORRECTIVE ACTION: This was corrected on-site. <br /> This is(Minor)Violation. <br /> Overall Inspection Comments: <br /> Routine Monitoring System Certification <br /> Complete and submit a copy of the Return to Compliance Certification form to the EHD with a statement <br /> documenting the corrective actions that have been or will be taken for each violation, and any supporting <br /> paperwork, by April 19, 2025. <br /> To minimize person to person contact EHD is choosing to write the name of person receiving the report instead of <br /> having them sign. <br /> Starting July 1, 2024, all in-office CERS help will be provided at EHD hourly rate ($172). <br /> To schedule an appointment, please call (209)468-3420. <br /> FA0017819 PR0526335 SCO01 03/20/2025 <br /> EHD Rev.12/06/2021 Page 6 of 7 CP22-UST FULL OIR <br /> 1868 E.
