Environmental Health Department <br />CP22 - HMBP Inspection Report <br />Date: <br />December 02, 2024 <br /> Facility Address: <br /> 1172 E YOSEMITE AVE, MANTECA <br /> Facility Name: <br /> SUBWAY <br />SUMMARY OF VIOLATIONS <br />(CLASS I, CLASS II, or MINOR - Notice to Comply) <br />RemarksItem # <br />b) Any handling of a previously undisclosed hazardous materials <br />c) A change of business or facility address <br />d) Change business ownership <br />e) Change of business name <br /> f)(1) A substantial change in the handler 's operations occurs that requires modification to any portion of the business <br />plan. <br />(2) For the purpose of this subdivision, “substantial change” means any change in a facility that would inhibit <br />immediate response during an emergency by either site personnel or emergency response personnel, or that could <br />inhibit the handler's ability to comply with Section 25507 , change the operational knowledge of the facility, or impede <br />implementation of the business plan. <br />CORRECTIVE ACTION: Immediately log into the California Environmental Reporting System (CERS) at <br />http://cers.calepa.ca.gov/, enter the correct or updated information, and submit to the EHD for approval. <br />This is (Class 2) Violation. <br />CA Health and Safety Code 25505(a)(3) 1010010 - Electronically submitted adequate response plan/procedures for <br />release/threatened release of hazmat. <br />OBSERVATION: The emergency response (ER) procedures were lacking or inadequate. The facility has a new <br />emergency contact, as the old one has no longer been there since 2021. <br />REGULATION GUIDANCE: A business plan shall include the following emergency response procedures for a release <br />or threatened release of hazardous materials, including, but not limited to, the following: <br />- immediate notification of local emergency personnel and the Environmental Health Department (EHD) <br />- procedures for the mitigation of a release or threatened release to minimize any potential harm or damage to <br />persons, property, or the environment <br />- evacuation plans and procedures, including immediate notice, for the site <br />CORRECTIVE ACTION: Immediately log into the California Environmental Reporting System (CERS) at <br />http://cers.calepa.ca.gov/, upload the correct or updated information, and submit to the EHD for approval. <br />This is (Class 2) Violation. <br /> 12 <br />CA Health and Safety Code 25505(a)(4) 1020001 - Established and electronically submitted adequate training <br />program. <br />OBSERVATION: After speaking with the new owner, Amandeep Kaur Sahota, a training program, for the handling of <br />hazardous materials, has not been implemented. <br />REGULATION GUIDANCE: Training programs for all employees and annual training , including refresher courses, shall <br />include familiarity with the facility's Emergency response plans and procedures in the event of a release or threatened <br />release of a hazardous material.
