The exhaust hood will be evaluated during the final inspection — ensure no roll-out. <br />All entry doors must self-close. <br />All construction must be completed according to the approved plans. Retain one copy of the approved <br />drawings on site during construction and bring with this unit for the inspection. Any change(s) to the <br />drawings must be submitted as revisions to this department. Revisions after approval are subject to <br />revision fee(s) (PC13). The applicant is responsible to contact this office to arrange a final inspection. <br />A completed Mobile Food Facility Permit to Operate Application Packet must be provided to the <br />undersigned before an inspection can be scheduled. Once the packet is submitted, contact the <br />undersigned to schedule the inspection. If more than one inspection is required, additional fees will <br />apply. <br />Absolutely no operation of the mobile food facility, food storage, or food preparation is allowed on the <br />unit until this Department issues your Permit to Operate. A permit will be issued upon the successful <br />review of the submitted application and the conclusion of a final inspection. <br />This food establishment will be required to have either the owner or an employee successfully pass an <br />approved and accredited food safety certification examination. This certification must be obtained within <br />sixty (60) days after your Permit to Operate is issued. Contact this Department for more information. <br />Expiration of Approval Notice: This project approval shall expire in 180 days by limitation, and become <br />null and void if the construction, reconstruction, alteration, or other work authorized by this approval has <br />not commenced within 180 days from the date of this notice. Once expired, before the work can be <br />recommenced, a new approval or extension must be obtained. New approvals must be requested in writing, <br />must not be altered from the initial approval, and are subject to the reactivation fee (PC96). This office will <br />discard/destroy the submitted plans two years after this approval date if the project has not been completed <br />or inspected in that time. Once plans are destroyed/discarded, a resubmittal fee (PC13) will be assessed <br />to the project and you will have to provide us the DEH stamped job copy to reactivate the project.
