Because <br /> when we spoke on the phone you stated that you were just going off of my email but my email lists 3 different <br /> inspection reports that have open violations. Please see comments below on your questions to#20 and#22 <br /> PR0540837—Aboveground Petroleum Storage Act <br /> #20: Page 11 of the SPCC Plan lists the procedures on what to report.The procedures are missing: <br /> - If damages or injuries occurred <br /> - If any evacuations occurred <br /> - Names of individuals/organizations that have been contacted <br /> #22: Page 12 of the SPCC Plan (Section Potential Equipment Failures—40 CFR 112.7(b)) lists Potential Spill from <br /> equipment failures.40 CFR 112.7(b) requires the plan to list prediction of direction of spill, rate of flow, quantity of <br /> oil that could be discharged as a result of each major equipment failure.The table on page 12 and 13 lists the <br /> direction of flow for Complete Tank Failure, Patrial Failure of Tank as"N/A—Contained in Secondary <br /> Containment". This does not meet the requirements in the regulations.Table 1 on Page 8 of the SPCC Plan also <br /> states the direction of flow is"Contained". Per the regulations, a prediction of the direction of flow needs to be <br /> included in the plan. Stating"contained" does not suffice.A prediction of the direction of flow should be listed in <br /> case of complete or partial failure of the tank. <br /> In addition,the Valve Leak also lists"N/A—Contained in Secondary Containment but that does not meet the <br /> requirements in the regulations and Tank Overfill, Pipe Failure, and Delivery Truck Hose Leak During Filling both <br /> refer to Figure 2.
