The most recent financial <br /> responsibility documents in CERS are dated 12/22/22. <br /> REGULATION GUIDANCE: Current financial responsibility documents are required to be submitted annually. <br /> CORRECTIVE ACTION: Immediately log into the California Environmental Reporting System (CERS)at <br /> http://cers.calepa.ca.gov/, and upload the required documents. <br /> OBSERVATION: Complete current financial responsibility documents were not found on site. On-site copy had a blank <br /> annual aggregate in Section A. <br /> REGULATION GUIDANCE:A copy of current financial responsibility documents are required to be maintained on site. <br /> CORRECTIVE ACTION: Immediately obtain a copy of the facility's financial responsibility documents and maintain a <br /> copy on site. <br /> This is a repeat violation (Class 2) Violation. <br /> 56 CA Code of Regulations 2716(a-e)2030013-DO inspections completed at least every 30 days and meet all <br /> requirements(TCR 12). <br /> OBSERVATION:The designated operator inspections for the following months were performed more than 30 days after <br /> the previous inspection: <br /> -Initial inspection on 5-5-23, next inspection performed on 6/5/23(31 days). <br /> REGULATION GUIDANCE:The designated operator visual inspections shall be conducted within 30 days or less of <br /> the previous inspection. <br /> CORRECTIVE ACTION: Ensure that all designated operator inspections are performed within the required time frames. <br /> ---------------------------------------------------------------------------------------------------- <br /> OBSERVATION:The designated operator failed to check and note all required items on the visual inspection reports. <br /> The incorrect and missing information included the following from the inspection reports: <br /> -Overfill test date listed in section 10 is incorrect <br /> -Second 87 spill container is missing from section 9 <br /> -Section 5 left blank <br /> -DO did not verify that the 12/21/23 alarm was responded to appropriately <br /> FA0021617 PR0537549 SCO01 06/20/2024 <br /> EHD Rev.12/06/2021 Page 8 of 10 CP22-UST FULL OIR <br /> 1868 E.
