A dOAQU1N Environmental Health Department <br /> CQLJIVTY AMENDED <br /> 6/11/2025 <br /> CP22 - UST FULL Inspection Report <br /> Facility Name: F255 <br /> ility Address: Date: <br /> HARNEY LANE AM PM E Harney LN, Lodi June 05 2025 <br /> SUMMARY OF VIOLATIONS <br /> (CLASS I,CLASS II,or MINOR-Notice to Comply) <br /> Item# Remarks <br /> OBSERVATION: Brine was found in the 87 fill sump, diesel fill, 91 STP, and 91 fill sump indicating a leak in the <br /> secondary containment. <br /> - Rubber seal was found to be missing on the lid of the 91-fill sump and ripped on the lid of the diesel fill sump. <br /> REGULATION GUIDANCE: Secondary containment shall be impervious to the liquid and vapor of the substance <br /> contained and constructed to prevent structural weakening as a result of contact with any hazardous substances <br /> released from the primary containment. <br /> CORRECTIVE ACTION: Immediately contact a properly licensed,trained, and certified contractor to make repairs to <br /> the UST system under permit and inspection of the EHD. <br /> This is a repeat violation (Class 2) Violation. <br /> 50 CA Code of Regulations 2635, 2637.1, 2665, 2712 2060020-Spill buckets meet requirements, have been tested as <br /> required, and records kept for 36 mths(TCR 9a). <br /> OBSERVATION:The two 87, 91, and diesel spill containers failed when tested. <br /> REGULATION GUIDANCE:All spill containers shall have a minimum capacity of five gallons and be capable of <br /> containing a spill or overfill until it is detected or cleaned up. <br /> CORRECTIVE ACTION: Immediately contact a properly licensed,trained, and certified contractor to repair or replace <br /> the spill container under permit and inspection of the EHD. <br /> This is a repeat violation (Class 2) Violation. <br /> 59 CA Code of Regulations 2712(b)(1-2)2030001 - Records of release detection monitoring and maintenance properly <br /> maintained (TCR 9d). <br /> OBSERVATION: Maintenance and monitoring records for the last three years were not found on site. <br /> -The L9 UDC 5/6 brine sensor had a low liquid alarm on 12/21/23, and 1/4/2025 indicating a loss of brine. No records <br /> were found on site addressing a response to the brine loss. <br /> - L18 premium/diesel annular sensor had a high liquid alarm on 4/30/2025,4/26/2025, and 4/22/2025 indicating liquid <br /> in the annular space. No records were found on site addressing a response to the high liquid. <br /> REGULATION GUIDANCE:These records shall be maintained on site for at least three years. Monitoring records <br /> include: (1)date and time of all monitoring or sampling; (2)monitoring equipment calibration and maintenance records; <br /> (3)results of any visual observations; (4)results of sample analysis performed a lab or in the field; (5)logs of all <br /> readings of gauges or other monitoring equipment, ground water elevations, or other test results; (6)results of <br /> inventory readings and reconciliations. <br /> CORRECTIVE ACTION: Immediately locate and maintain all missing maintenance and monitoring records for the last <br /> three years on site.
